摘要
目的探讨手术治疗老年结直肠癌患者急性肠梗阻的疗效。方法选取2005年11月至2009年10月间收治的75例老年结直肠癌合并急性肠梗阻患者,均为急诊外科手术治疗,观察治疗后的临床效果、并发症和生存情况。结果术后死亡4例,发生率为5.3%。术后并发症主要为感染,占12.0%。患者手术前和手术后、手术前和随访时的腹痛腹胀、大便习惯性改变、腹泻和便秘交替、便血、腹膜刺激征、呕吐、停止排气的差异均有统计学意义(P<0.05)。术后6个月的生存率为92.0%,术后1年的生存率为86.7%,术后3年的生存率为54.7%,术后5年的生存率为26.7%。出院时Karnofsky体力状况(KPS)评分总有效率为83.1%,随访时为73.9%,差异有统计学意义(P<0.05)。结论手术治疗老年结肠癌并急性肠梗阻预后良好,近期生存率高。
Objective To investigate the clinical efficacy of elderly patients with colorectal cancer combined with acute intestinal obstruction by surgery. Methods Seventy-five cases with colorectal cancer combined with acute intestinal obstruction from November 2005 to October 2009 were chosen,who were given emergency surgery and the clinical efficacy,complications and survival situation were observed. Results The death was 4 cases,the incidence was 5. 3%,the mainly complications was postoperative infection（ 12. 0%）,the abdominal pain,bloating changes in bowel habits,alternating diarrhea and constipation,blood in the stool,peritoneal irritation,vomiting,stopping venting before and after surgery,before surgery and follow-up had significant differences（ P〈0. 05）; the survival rate after surgery was 94. 7%,after surgery for 6 months was 92. 0%,for 1-year was 86. 7%,for 3-year was 54. 7%,for 5-year was 26. 7%,the total efficacy of Karnofsky performance status（ KPS） score at discharge was 83. 1%,at follow-up was73. 9%,which had significant difference（ P〈0. 05）. Conclusion The elderly colorectal cancer combined with acute intestinal obstruction by surgery had good prognosis,and the recent survival rate was high.
