摘要
目的:本文对体外循环及非体外循环下冠状动脉旁路移植术(CABG)治疗冠心病(CHD)的效果进行比较。方法:选取我院2017年2月～2018年3月所收治的122例CABG的CHD患者作为研究对象,随机分为对照组和观察组,对照组给予体外循环下CABG治疗,观察组给予非体外循环下CABG治疗,比较两组患者的治疗效果。结果:根据研究结果显示,观察组总有效59例(96.72%),对照组总有效49例(80.32%),差异具有统计学意义(P<0.05)。观察组手术时间、术中输血量、重症监护病房治疗时间和住院时间均短于对照组,差异均有统计学意义(均P<0.05)。两组患者治疗后生活质量评分和治疗前相比均有所改善(均P<0.05),且观察组患者术后生活质量明显优于对照组(P<0.05)。两组患者术前心肌酶指标无统计学差异,术后24h观察组心肌酶明显高于对照组,差异有统计学意义(P<0.01)。观察组术后并发症发生率为1.6%(1/61),显著低于对照组13.1%(8/61),差异有统计学意义(P<0.05)。结论:针对CHD患者,非体外循环下CABG治疗可以有效提高治疗效果,减少术后并发症的发生,缩短住院时间,并有助于改善患者术后生活质量,值得临床推广应用。
Objective:To evaluate the efficacy of coronary artery bypass grafting(CABG)in the treatment of coronary heart disease(CHD)under cardiopulmonary bypass(CPB)or off-pump.Methods:122 CABG patients admitted in our hospital from February 2017 to March 2018 were randomly divided into control and observation group.The control group was treated with CABG under CPB,and the observation group was treated with CABG under off-pump,respectively.Results:According to the results of the study,59 cases(96.72%)were effective in the observation group and 49 cases(80.32%)in the control group,with a significant difference between the two groups(P<0.05).The length of operation,volume of intraoperative blood transfusion,treatment duration in intensive care unit(ICU)and hospital stay were significantly shorter in observation group than those in control group(all P<0.05).Compared with the control group,the quality of life in the observation group was better than that in control group(P<0.05).There was no significant difference between the two groups in myocardial enzyme before operation.While the myocardial enzyme level was raised at 24 hafter operation in two groups,and significantly higher in observation group than that in control group(P<0.01).The incidence of postoperative complications in the observation group was 1.6%(1/61),which was significantly lower than 13.1%(8/61)in the control group(P<0.05).Conclusion:CABG without extracorporeal circulation can effectively improve the therapeutic effect,reduce the incidence of complications after operation,and shorten hospital stay,which is helpful to improve the postoperative quality of life and worthy of clinical application.
