摘要
目的:探讨异位胃黏膜显像在小儿梅克尔憩室诊断中的应用价值。方法:对临床怀疑有梅克尔憩室的40名患儿进行异位胃黏膜显像,以术后病理、胃肠镜检查及临床随访结果为确诊依据,分析患儿的临床特征和显像特点,计算异位胃黏膜显像在梅克尔憩室诊断中的灵敏度、特异度及准确度。结果:5例显像为阳性,均通过术后病检确诊为梅克尔憩室。35例显像为阴性,其中34例通过病检或其他辅助检查方法确诊为其他疾病,另有1例通过术后病检确诊为梅克尔憩室。梅克尔憩室患儿临床表现以便血和腹痛为主,异位胃黏膜显像表现为类圆形或条片状的显像剂浓聚影。异位胃黏膜显像在小儿梅克尔憩室诊断中的灵敏度为83%(5/6)、特异度为100%(34/34)、准确度为97.5%(39/40)。结论:异位胃黏膜显像安全无创、简便易行、灵敏度、特异度及准确度均较高,在梅克尔憩室的诊断中具有很高的应用价值。
Objective:To evaluate the diagnostic efficacy of ectopic gastric mucosal imaging in children with Meckel’s diverticulum.Methods:The ectopic gastric mucosal imaging was performed on 40 pediatric patients with clinical suspicion of Meckel’s diverticulum.Pathological results,gastrointestinal endoscopy results and clinical follow-up were taken as the gold standard.The clinical and imaging characteristics were analyzed,the sensitivity,specificity and accuracy of ectopic gastric mucosal imaging in diagnosis of Meckel’s diverticulum were also calculated.Results:5 cases were positive and were diagnosed as Meckel’s diverticulum by pathological detection.The rest were negative.34 cases of them were diagnosed as other diseases by pathological detection or other auxiliary examination.However,one case was diagnosed as Meckel’s diverticulum by pathological detection.The main clinical characteristics of Meckel’s diverticulum were hematochezia and stomachache.The ectopic gastric mucosal imaging showed in shape of quasi-circular and flake.The sensitivity of ectopic gastric mucosal imaging in diagnosis of Meckel’s diverticulum in children was 83%(5/6),the specificity was100%(34/34),and the accuracy was 97.5%(39/40).Conclusion:Ectopic gastric mucosa imaging is a safe and simple method with high sensitivity,specificity and accuracy,which plays an important role in diagnosis of Meckel’s diverticulum.
