摘要
目的探讨心脏术后使用无创呼吸机是否给病人带来益处。方法采用病例回顾性分析,评估使用无创呼吸机和有创呼吸机的氧分压、心率、住院时间差异。分无创机械通气组45例,有创机械通气组26例,无创机械通气根据病人的情况有两种模式可选择,即DUOPAP和NIV模式。结果给予辅助通气前,2组患者的心率及PO2差异无统计学意义(P>0.05);给予辅助通气后,无创通气组患者的心率由T1(125.5±20.30)次/min,下降到T2(123.60±20.36)次/min、T3(104.30±11.8)次/min、T4(89.40±9.25)次/min;PO2由T1(60.70±14.12)mm Hg,上升到T2(118.30±29.2)mm Hg、T3(150.40±26.3)mm Hg、T4(130.90±14.3)mm Hg。有创通气组患者的心率由T1(135.27±21.29)次/min,下降到T2(115.2±18.03)次/min、T3(103.00±10.9)次/min、T4(88.00±12.70)次/min;PO2由T1(62.45±11.56)mm Hg上升到T2(121.55±29.50)mm Hg、T3(137.8±24.77)mm Hg、T4(131.6±17.00)mm Hg。2组患者心率及PO2均较辅助通气前降低(P<0.05);2组患者间各时间点心率及PO2比较差异无统计学意义(P>0.05)。无创通气组的ICU停留时间及术后至出院时间分别为(3.8±2.8)d及(8.2±4.3)d,均低于有创通气组的(7.38±5.13)d及(15.00±0.92)d,差异有统计学意义(P<0.05)。结论在心脏外科术后,对有适应证的病人尽早使用无创正压通气益处大,建议推广应用。
Objective To investigate whether noninvasive ventilator can be benefit for the cardiac patients. Methods The oxy- gen partial press （ PO2 ）, heart rate and hospital stays were observed between noninvasive and invasive mechanical ventilation by retrospec- tive study. There were 45 cases in noninvasive mechanical ventilation group and 26 cases in invasive mechanical ventilation group. Nonin- vasive mechanical ventilation had two modes including DUOPAP and NIV. Results Before assisted ventilation, there was no statisticaldifference between the two groups in heart rate and PO2. The blood gas analysis, oxygen partial pressure, ICU staying time and postopera- tive time were recorded and compared at the time of need for mechanical ventilation （ T1 ）, after 30 minutes of mechanical ventilation （T2）, after 2 hours of mechanical ventilation （T3 ） and before the patients were presumed to depart from mechanical ventilation （ T4 ）. Conclusion The patients in cardiac postoperative surgery are benefit from noninvasive positive pressure ventilation , and suggest that we should apply it universal.
