摘要
目的研究血清超敏c-反应蛋白水平（CRP）对临产后剖宫产产妇早期感染的预测价值。方法选取2011年5月到2015年5月期间来衡水市第二人民医院进行临产后剖宫产的产妇215例，选取其中发热产妇30例，作为观察组，选取其余185例无发热症状的产妇作为对照组，比较两组产妇临产之前，术后当天以及术后2、4、6天检测血清CRP的水平。结果对两组产妇不同时间段血清的CRP进行比较，发现临产之前，手术当天两组产妇血清的CRP水平无显著性差异（t值分别为0．30、0．69，均P〉0．05），手术后2、4、6天，观察组产妇血清的CRP水平均显著高于对照组产妇（t值分别为4．10、4．32、2．85，均P〈0．05）。对两组产妇术后血清CRP的下降量进行比较，发现术后2—4天对照组产妇血清CRP的下降量显著高于观察组产妇（t=4．01，P〈0．05），术后4—6天两组产妇的下降量无显著性差异（t=1．13，P〉0．05）。血清cI冲水平在预测产妇剖产后早期感染有着显著的价值，其敏感性76％．特异性60％，阳性预测值68％，阴性预测值70％，感染总符合率皆高于60％。结论血清CRP对临产后剖宫产产妇早期感染有着显著的预测价值，对产妇早期感染的判断有重要意义。
Objective To study the predictive value of serum C-reactive protein （CRP） on early maternal infection after cesarean delivery in labor. Methods From May 2011 to May 2015, 215 cases of cesarean section in labor were selected in Hengshui Second People＇ s Hospital, including 30 cases with fever in observation group and 185 cases without fever symptom in control group. CRP level before labor, at surgery day, 2, 4, and 6 day after surgery was compared between two groups. Results Comparison between two groups showed that there was no significant difference in CRP level before surgery and at surgery day （t value was 0.30 and 0.69, respectively, both P 〉 0.05 ）, but CRP level at 2, 4 and 6 days after surgery was Significantly higher in the observation group than in the control group （ t value was 4.10, 4.32 and 2.85, respectively, all P 〈 0.05）. Decrease of CRP level 2 - 4 days after surgery in the control group was significantly greater than in the observation group （ t = 4.01, P 〈 0.05 ）, but that 4 - 6 days after surgery was not statistically different between two groups （ t = 1.13,P 〉 0.05 ）. Serum CRP level had important value in predicting early infection after cesarean section, and the sensitivity, specificity, positive predictive value, negative predictive value and infection coincidence was 76%, 60%, 68%, 70% and 60%, respectively. Conclusion Serum CRP has obvious predictive value for early maternal infection after cesarean delivery, and it is essential in judging early maternal infection.
