实时三维超声评估负荷过重患者右心室收缩功能 被引量:1

Assessment of right ventricular function in patients with right ventricular overload using real-time three-dimensional echocardiography

摘要 目的运用实时三维超声心动图(RT-3DE)评估容量和压力负荷过重右心室整体及节段收缩功能。方法选取右室容量负荷过重患者(A组)、压力负荷过重患者(B组)及正常成人(C组)各45例行RT-3DE检查。应用TomTec分析软件获得右室整体及三个节段舒张末期容积(EDV)、收缩末期容积(ESV)、每搏量(SV)、射血分数(EF)并进行组间比较。同时分析三维右室EF与二维参数以及右室收缩压(RVSP)的相关性。结果与对照组比较,A、B组整体EDV、ESV均增大(P〈0.05),组间差异无统计学意义(P〉0.05);A组整体SV代偿性增高(P〈0.05),B组整体SV与对照组差异无统计学意义(P〉0.05);两组整体EF均降低(P〈0.05),B组较A组降低(P〈0.05)。A组SV代偿性增高表现在流人道和体部(P〈O.05),两个节段EF与对照组差异无统计学意义(P〉0.05),流出道SV代偿不明显(P〉0.05),EF降低(P〈0.05)。B组各节段SV均未见代偿性增高(P〉0.05),各节段EF均降低(P〈0.05)。压力负荷过重组RVSP与右室整体及节段EF均无直线相关。结论RT-3DE能有效评估容量或压力过负荷右室整体及节段收缩功能。 Objective To evaluate global and segmental right ventricular (RV) systolic functions in patients with excessive volume or pressure load using real-time three-dimensional echocardiography(RT- 3DE). Methods Forty-five patients with RV volume overload, 45 patients with RV pressure overload and 45 healthy subjects were underwent RT-3DE. RV global and segmental (inflow, body, outflow) end- diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV) and ejection fraction (EF) were analyzed with TomTec software. The correlations between EF with the three-dimensional method and two- dimensional parameters including right ventricle systolic pressure(RVSP) were discussed. Results Global EDV and ESV increased significantly in both patient groups compared with controls (all P 〈 0.05), but there was no difference between two patient groups ( P 〉0.05). Compensated increase of SV was found in sixty percent of patients with volume overload but none with pressure overload ( P 〈0.05). Global EF decreased significantly in both of patient groups (all P 〈0.05), which was more significant patients with pressure overload (P 〈 0.05). Different patterns of the regional dysfunction were found among the different RV segments. No correlation was found between RVSP and global or segmental EF in patients with pressure overload. Conclusions RT-3DE could be used to assess global and segmental RV systolic function in patients with pressure and volume overload.
