摘要
目的:评估晚期非小细胞肺癌患者接受冷冻消融序贯化疗后的肿瘤恶性程度。方法:选择接受冷冻消融序贯化疗和接受单纯化疗的晚期非小细胞肺癌患者(各39例),分别纳入序贯组和对照组,随访两组患者的疾病进展情况以及生存情况,检测血清中肿瘤标志物、血管新生分子的含量以及外周血中T淋巴细胞亚群的含量。结果:序贯组患者的无进展生存期(PFS)以及中位生存总生存期(mOS)长于对照组,各个时间点处的肿瘤累积进展例数均显著少于对照组;治疗后1个月时,序贯组患者血清中肿瘤标志物癌胚抗原(CEA)、细胞角蛋白19(CYFRA21-1)、神经特异性烯醇化酶(NSE)的含量、血管新生分子畸胎瘤细胞衍生生长因子(PCDGF)、血管内皮生长因子(VEGF)、肝癌衍生生长因子(HDGF)的含量以及外周血中CD3^+CD4^-CD8^+CD28^-T细胞含量均显著低于对照组,外周血中CD3^+CD4^+CD8^-T细胞、CD3^+CD4^-CD8^+CD28^+T细胞的含量高于对照组。结论:冷冻消融序贯化疗能够改善晚期非小细胞肺癌患者的预后情况,延缓病情进展、延长生存时间、抑制血管新生、增强免疫功能。
Objective: To evaluate the tumor malignancy in patients with advanced non-small cell lung cancer after cryo- ablation sequential chemotherapy. Methods: A total of 78 cases of patients with advanced non-small cell lung cancer who re- ceived cryoablation sequential chemotherapy and chemotherapy alone, with 39 cases in each, were selected and enrolled in se quential group and control group. Disease progression and survival of two groups were followed up, and contents of tumor markers and angiogenesis molecules in serum as well as contents of T-lymphocyte subsets in peripheral blood were detected. Results: Progression-free survival (PFS) and median overall survival (mOS) of sequential group were longer than those of con- trol group, and cumulative cases of tumor progression at various points in time were significantly less than those of control group; 1 month after treatment, serum tumor markers CEA, CYFRA21-1 and NSE contents, serum angiogenesis molecules PCDGF, VEGF and HDGF contents as well as CD3+ CD4 CD8+ CD28 T cell content in peripheral blood of sequential group were significantly lower than those of control group, and contents of CD3+ CD4+ CD8 T cell and CD3+ CD4 CD8+ CD28+ T cell in peripheral blood were higher than those of control group. Conclusion: Cryoablation sequential chemotherapy can improve theprognosis of patients with advanced non-small cell lung cancer, delay disease progression, prolong survival time, inhibit angio- genesis and improve immune function.
