摘要
目的探讨鼻内镜辅助U型管植入治疗儿童泪小管断裂伤的临床效果。方法选择2011年1月至2014年12月收治儿童泪小管断裂88例患者,采用在鼻内窥镜下U形植入泪道引流管吻合泪小管断端,留置引流管3~6月,拔管3~9个月随访观察疗效。结果 85例（96.6%）泪小管断裂吻合成功,泪道冲洗通畅,拔管顺利,无溢泪。3例（3.4%）冲洗不通畅,经激光成形再次装管后冲洗通畅。所有儿童及家属对外观满意,舒适度良好。结论鼻内窥镜下双头U环形植入泪道引流管治疗儿童泪小管断裂的临床效果较好,可减少医源性损伤,不影响患者的外观及生活质量,值得临床推广。
Objective To evaluate the clinical efficacy of the treatment to children lacrimal duct rupture with U shaped tube insertion assisted by a nasal endoscope. Methods 88 cases of pediatric patients who were admitted to hospital because of lacrimal duct rupture from Jan,2011 to Dec,2014 were treated with the insertion of lacrimal drainage tube assisted by a nasal endoscope to anastomose with the break end of lacrimal duct. The drainage tube were left in lacrimal duct for 3 to 6 months and followup were conducted to observe the efficacy after taking out the drainage tube for 3 to 9 months. Results 85（ 96. 6%） cases had a successful anastomosis with unimpeded flushing of lacrimal passage and successful taking out of drainage,and without tear flowing. 3（ 3. 4%） cases did not have a unimpeded flushing until being given a laser forming retubulation. All children and their families were satisfied with its appearance and favorable degree of comfort. Conclusion Treatment to children lacrimal duct rupture with the insertion of dicephalous U shaped lacrimal drainage tube assisted by a nasal endoscope has a good clinic efficacy,which is deserve a popularization as it can reduce iatrogenic injury and does not influence patients＇ appearances and life quality.
