摘要
目的 探讨超声造影定量分析技术在评价类风湿性关节炎(RA)患者治疗前后滑膜微血流灌注改变的临床价值,通过比较时间-强度曲线的定量参数治疗前后的变化,获得这些参数中能评价RA治疗效果的敏感指标.方法 收集我院2015年1月至10月风湿免疫科确诊为类风湿性关节炎的患者30例,均符合1987年美国风湿病协会(ARA)修订RA诊断标准,治疗前和治疗一个疗程(3个月)后均进行活动性评分(DAS28)和超声造影检查.根据治疗前后DAS28的变化,将患者分为治疗有效组(DAS28降低0.6或以上)和治疗无效组(DAS28降低在0.6以下或DAS28仍然在5.1以上),通过比较这两组超声造影时间—强度曲线定量参数如曲线下面积(AUC)、峰值强度(PI)、达峰时间(TTP)、平均通过时间(MTT)等的变化,从而得出这些参数中能评价RA治疗效果的敏感指标.结果 30例RA患者治疗前后效果显示,治疗有效组峰值强度下降值为(2.7±1.1),大于治疗无效组的(1.4±1.8),P< 0.05.结论 超声造影定量分析技术可敏感反映增生滑膜治疗前后微血管血流灌注的变化特点,可作为定量评价RA活动性的手段,超声造影时间-强度曲线定量参数中的峰值强度可以成为评价RA活动性及治疗效果的敏感指标.
Objective To evaluate the clinical value of quantitative analysis of contrast-enhanced ultrasound in patients with rheumatoid arthritis (RA) before and after treatment and to obtain sensitive indicators evaluating RA comparing the changes of the quantitative parameters of time intensity curve.Methods 30 patients diagnosed with RA at our hospital from January, 2015 to October, 2015 were collected.Ultrasonic checking and activity scoring were taken before the treatment and after 1 treatment cycle (3 months).The patients were divided into an effective group (DAS28 decreased 0.6 or more) and an effectless group (DAS28〉 5.1 or DAS28 decreased less than 0.6) according the change of DAS28.Parameters such as area under the curve (AUC), peak intensity (PI), peak time (TTP), and average pass time (MTT) were obtained.These parameters were sensitive to RA treatment.Results The change of peak intensity was (2.7±1.1) in the effective group and (1.4±1.8) in the effectless group, with a statistical difference (P 〈 0.05).Conclusions Quantitative analysis technique of contrast-enhanced ultrasound is sensitive to the changes of blood flow in the patients before and after the treatment.It can be used as a means of quantitative evaluation of RA activity.The peak intensity of the time intensity curve quantitative parameters can be used as a sensitive index to evaluate RA activity and treatment effect.
