摘要
目的探讨CT血管造影(CTA)在急性脑血管病中的诊断价值。方法选取2012年2月-2015年2月于我院治疗的78例急性脑血管疾病患者为研究对象,对其临床资料进行回顾性分析。所有患者入院后均行CTA检查,采用最大密度投影(MIP)与表面阴影成像技术(SSD)对所获得的图像进行三维重建,并将CTA检查结果与数字减影血管造影(DSA)或手术所见结果相比较,评价CTA对急性脑血管疾病的诊断价值。结果 CTA对急性脑血管病诊断的准确率为96.15%,对自发性颅内出血、脑缺血性疾病、脑血管畸形诊断的准确率高达100.0%,对颅内动脉瘤诊断的准确率也较高,为90.63%(29/32),CTA对急性脑血管病的诊断结果与DSA或手术所见结果大致相同,两者比较,差异无统计学意义(p>0.05)。结论 CT血管造影对急性脑血管病具有较高的诊断率,可作为神经内科诊断脑血管疾病的首选影像学检查方法,值得临床推广。
Objective To explore the diagnostic value of CT angiography(CTA) in acute cerebrovascular disease. Methods Seventy eight patients with acute cerebrovascular disease treated in the Hospital from February, 2012 to February, 2015 were selected as research subjects, and the clinical data were retrospectively analyzed. All the patients underwent CTA examinations upon their admission, maximum intensity projection and surface shadow imaging technologies were applied to conduct three-dimensional reconstruction on images obtained, and CTA inspection results were compared to those obtained via digital subtraction angiography(DSA) or results found in surgeries to evaluate the diagnostic value of CTA on acute cerebrovascular disease. Results The accuracy of CTA in the diagnosis of acute cerebrovascular disease was 96.15%, and its accuracy on spontaneous intracranial hemorrhage, cerebral ischemic disease and cerebrovascular malformation hit 100%, and it also delivered high accuracy in the diagnosis of intracranial aneurysm at 90.63%(29/32), results of CTA in the diagnosis of acute cerebrovascular disease were roughly same to that shown in DSA or surgeries, and the differences were not statistically significant(P〈0.05) when two of them were compared. Conclusion Conclusion CT angiography delivers diagnostic rate on acute cerebrovascular disease, and it can be used as preferred imaging examination method for diagnosis of cerebrovascular diseases in the department of neurology, be worthy of clinical promotion.
