摘要
目的比较分析内镜窄带成像技术(NBI)联合放大内镜对内镜下可疑Barrett食管(BE)行靶向活检方法与传统四象限活检法对BE活检的阳性率。方法将2012年1月至2015年1月该院收治的经胃镜检查诊断为内镜下可疑BE患者300例分为NBI组和对照组,各150例。对照组采用传统四象限活检法在普通内镜下对病变处进行活检,NBI组采用NBI联合放大内镜下对病变处进行靶向活检,比较2种方法BE活检的阳性率。结果 NBI组活检阳性率达82.00%(123/150),而对照组为67.33%(101/150),两组比较,差异有统计学意义(P<0.01);NBI组可疑岛型、舌型、全周型黏膜BE患者活检病理柱状上皮检出率均高于对照组,差异有统计学意义(P<0.01)。结论 NBI联合放大内镜对病变轮廓、BE黏膜腺管开口及浅表毛细血管结构形态显示更加清晰,因此靶向活检更有助于提高BE活检阳性率,具有较好的临床实用性,值得推广。
Objective To compare the positive rates between the targeted-biopsy and traditional four-quadrant biopsy by using narrow-band imaging combined with magnifying endoscopy （NBI-ME） in endoscopically suspected Barrett＇s esophagus （BE）. Methods A total of 300 patients with endoscopically diagnosed suspected BE in our hospital from January 2012 to January 2015 were divided into the NBI group and the control group, 150 cases in each group. The control group adopted the traditional fourquadrant biopsy method for conducting biopsy under the ordinary endoscopy,while the NBI group adopted the NB-ME for conducting the targeted biopsy. The positive rate of BE biopsy were compared between the two groups. Results The biopsy positive rate of the NBI group for diagnosing BE was 82.00% （123/150） ,while which in the control group was 67.33% （101/150） ,the dif ference between the two groups was statistically significant （P〈0.01） ;the diagnosis coincidence rates of island-type, tongue-type and complete cycle type BE in the NBI group were higher than those in the control group with statistical difference （P〈0.01）. Conclusion NBI-ME can display the lesion outline,opening of mucosal duct and superficial capillary structural morphology in BE more clearly, therefore the targeted biopsy is more conducive to increase the positive rate of BE biopsy, has better clinical practicability and is worthy of being promoted.
