摘要
目的观察关节镜下胫骨、股骨双隧道四股同种异体肌腱双束重建膝前交叉韧带的临床效果。方法选取(30个膝)膝关节前交叉韧带断裂患者30例,采用关节镜下胫骨、股骨双隧道四股同种异体肌腱双束重建膝前交叉韧带,术后1年测定患者的韧带紧张度,并且采用Lysholm评分、国际膝关节评分委员会(IKDC)量表、美国膝关节协会(AKS)量表对患者膝关节功能恢复效果进行评定。结果 30例患者韧带紧张度达优者25例(83.3%),良4例(13.3%),差1例(3.3%),优良率为96.7%。术后复查MRI均未发现关节间隙变窄、螺钉或移植物松动病例。术后Lysholm评分、IKDC评分、AKS评分均高于术前(P<0.05)。结论关节镜下胫骨、股骨双隧道四股同种异体肌腱双束重建膝前交叉韧带的操作简便,患者术后膝关节稳定性好,最大限度恢复至解剖重建,治疗效果理想。
Objective To observe the clinical efficacy of arthroscoped-guided double-bundle reconstruction of knee anterior cruciate ligament using four strands of allogeneic tendons through double tunnels in tibia and femur. Methods Thirty patients（ 30 knees） with anterior cruciate ligament rupture of knee joint were enrolled,and underwent arthroscoped-guided double-bundle reconstruction of knee anterior cruciate ligament using four strands of allogeneic tendons through double tunnels in tibia and femur. The ligament tension was detected one year after surgery,and the functional recovery of knee joint was evaluated by Lysholm Scale,International Knee Documentation Committee（ IKDC） Scale,American Knee Society（ AKS） Scale in all patients. Results In 30 patients,there were 25 with excellent ligament tension（ 83. 3%）,4 with good ligament tension（ 13. 3%） and 1 with poor ligament tension（ 3. 3%）,and the fine rate was 96. 7%. The postoperative MRI scan showed that no joint space narrowing,loose of screws or graft were found in any cases. Postoperative Lysholm score,IKDC score and AKS score were higher than the preoperative scores（ P 〈 0. 05）. Conclusion Arthroscoped-guided double-bundle reconstruction of knee anterior cruciate ligament using four strands of allogeneic tendons through double tunnels in tibia and femur is an approach with simple manipulation. After the surgery,the patients can acquire a good stability of knee joint,resume to anatomical reconstruction furthest and have an ideal therapeutic efficacy.
