摘要
目的分析严重Rh溶血病并发持续肺动脉高压(persistent pulmonary hypertension of newborn,PPHN)、肺出血的原因,以便早期诊断及治疗,降低病死率。方法回顾性分析严重Rh溶血病患儿并发PPHN、肺出血的临床病历资料,并对病因、发病机制进行文献复习。结果严重Rh溶血病、极重度贫血可以合并PPHN、肺出血,经一氧化氮吸入、高频通气及支持对症治疗好转。结论新生儿严重溶血病出现低氧性呼吸衰竭要注意PPHN,治疗过程中呼吸支持联合NO治疗是成功的保证,同时注意液体管理、心肺功能监测及血凝监测。早期诊断、综合治疗是提高生存率的关键。
Objective To investigate the causes of severe Rh hemolytic disease complicated with persistent pulmonary hypertension(PPHN)and pulmonary hemorrhage of newborn,so as to make early diagnosis and treatment,as well as reduce the mortality.Methods The clinical data of severe Rh hemolytic disease complicated with PPHN and pulmonary hemorrhage in children were retrospectively analyzed,and the mechanism of etiology and pathogenesis were discussed with literature review.Results Severe Rh hemolytic disease with extremely severe anemia can be complicated with PPHN and pulmonary hemorrhage,which can be improved by nitric oxide inhalation,high frequency ventilation and supporting symptomatictreatment.Conclusion PPHN should be considered when severe hemolytic disease in neonate was complicated with hypoxic respiratory failure,which can be successfully treated by respiratory support combined with NO.Cardiopulmonary function and coagulation should alsobe monitored during the process of treatment.Early diagnosis and comprehensive treatment are the key to improve the survival rate.
