摘要
目的探讨直肠癌患者行直肠低位双吻合微创外科的效果研究,为其临床应用提供可参考依据。方法选择175例行直肠低位双吻合手术治疗的直肠癌患者作为研究对象,其中85例在腹腔镜下完成,90例开腹完成。比较两组患者围手术期差异,并比较其对机体应激反应的差异。结果腹腔镜组术中出血量、手术时间、术后引流量、术后引流时间、肠鸣音出现时间、排气时间及住院时间均明显低于开腹组,差异有统计学意义(P<0.05)。腹腔镜组吻合口瘘、肠梗阻发生率明显低于开腹组,差异有统计学意义(P<0.05)。术后腹腔镜组患者CR P、IL-6和TN F-α水平均低于开腹组患者,差异有统计学意义(P<0.05)。结论腹腔镜下直肠低位双吻合治疗低位直肠癌患者有较好的临床疗效,在有效提高保肛率的前提下,可降低吻合口瘘及肠梗阻并发症的发生。
【Objective】 To observe and explore the significance of vertical cutting of the closed distal rectum in dual-anastomosis for patients with low rectal cancer. 【Methods】 In this study, 85 patients diagnosed as low rectal cancer given laparoscopic low intestinal dual-anastomosis treatment（laparoscopic group）and 90 patients of low rectal cancer given laparotomy low-bowel double match treatment（laparotomy group）were included. The intraoperative and postoperative differences and complications were compared between both groups. 【Results】 The intraoperative blood loss, operative time, postoperative bowel sounds, postoperative drainage time and volume, exhaust time and hospital stay in the laparoscopic group were significantly lower than those in the laparotomy group with significant differences（P 0.05）. The incidences of anastomotic fistula and intestinal obstruction in the laparoscopic group were significantly lower than that in the laparotomy group,the differences were statistically significant（P 0.05）. The levels of CRP, IL-6 and TNF-α in the laparoscopic group were significantly lower than those in the laparotomy group（P 0.05）. 【Conclusions】 Laparoscopic downward rectal low double match treatment has a better clinical effect in low rectal cancer patients, which can effectively reduce the incidences of anastomotic fistula and intestinal obstruction.
