摘要
目的:探讨肾上腺髓样脂肪瘤和肾上腺皮质腺瘤的CT表现以及诊断与鉴别诊断。方法:收集经手术后病理证实的肾上腺髓样脂肪瘤和肾上腺皮质腺瘤各17例的CT及临床、病理资料。结果:17例肾上腺髓样脂肪瘤,单右侧10例,单左侧7例,肿瘤呈椭圆形或圆形,边界清楚,形态为椭圆形9例,圆形8例,占55%、45%,肿瘤内密度不均匀,以脂肪为主15例,占88%,以软组织为主2例,占12%,增强扫描软组织轻中度较均匀强化,1例有条状钙化灶,占5%。17例肾上腺皮质腺瘤,单右侧10例,单左侧7例,肿瘤多呈类圆形,边界清晰,占100%,肿瘤内密度基本均匀,呈低密度液性密度影,增强后强化较均匀,轻中度强化。肾上腺髓样脂肪瘤最低CT值低于皮质腺瘤(P<0.05)。结论:肾上腺髓样脂肪瘤及皮质腺瘤的CT表现较具特征性,对肾上腺髓样脂肪瘤及皮质腺瘤诊断及鉴别诊断具有重要价值。
Objective： To investigate diagnostic and differential diagnostic value of using computed tomography for adrenal myelolipoma and adrenocortical adenoma. Methods： CT images and clinical and pathological data of 17 cases of adrenal myelolipoma and 17 cases of adrenocortical adenoma, confirmed by pathology, were collected and analyzed retrospectively. Results： Of all 17 cases of adrenal myelolipoma, 10 cases occurred on the right side only and 7 on the left side only. The plain CT showed heterogeneous density, fatty tissue was the main features in 15 cases（88%）, and soft tissue was the main features in 2 cases（12%）. CT features of lesions presented as either oval（9 cases） or round（8 cases） masses with smooth border and moderate homogeneous enhancement in enhanced scan, accounting for 55% and 45% respectively. The lesion showed strip calcification in 1case（5%）. The CT findings of lesions included well-defined margin, oval in shape, and homogeneous low fluid density,showing mild to moderate homogeneous enhancement in enhanced scan. The lowest CT value of adrenal myelolipoma was lower than that of adrenocortical adenoma. Conclusion： The results presented above showed that there are distinct characteristic appearances of adrenal myelolipoma and adrenocortical adenoma on CT, which is valuable in diagnosis and differential diagnosis of the two tumors.
