摘要
目的:利用循证医学的方法分析灸法临床实践中高度相关的高质量证据,为临床医生提供科学证据。方法:立足于灸法临床实践中常见的重要问题,全面检索当前可得的临床研究文献,按照改良循证医学5级标准从高到低选用能够回答对应临床问题的证据,采用Rev Man5.3统计结局指标。结果:有15篇符合要求的文献,2项1c级和1项2a级证据表明灸法治疗膝骨性关节炎的有效性,3项2b级和2项2c级证据表明灸法可能优于西药或者疗效相当。2项2a和1项3a级证据表明,不同灸法之间有疗效差异,其中以热敏灸疗效最佳。1项2b级证据比较了灸法结合西药与西药之间的疗效差异,并表明灸法结合西药优于单用西药。结论:灸法治疗膝骨性关节炎是有效的且安全副作用小,不同灸法疗效各异,针药并用的效果可能更佳,尚需高质量试验证实。
Objective： Using the method of evidence-based medicine to analyze highly relevant high quality evidence in moxibustion clinical practice,providing scientific evidence for clinical doctors. Methods： Centered on moxibustion clinical practices,we retrieved the current available clinical research literature comprehensively,and collected suitable key to the problem according with modified evidence grade in evidence-based medicine. The Cochrane Collaboration＇s Rev Man5. 3software was used for data analyses. Results： There are 15 studies in this research. Two1c-Level and one 2a-Level evidence confirmed the efficacy of moxibustion the treatment of Knee Osteoarthritis. Three 2b-Level and two 2c-Level evidence that the therapeutic effect of moxibustion may be superior or equal to western medicine. Two 2a-level and one 1a-level evidence indicate that different moxibustion have different effects. But the heat-sensitive moxibustion is the best. One 2b-Level evidence compared the differences in effectiveness between moxibustion combined with western medicine and western medicine,and show that moxibustion combined with western medicine is better than western medicine. Conclusion：Moxibustion treatment effectiveness of Knee Osteoarthritis is certain; Compared with western medicine,moxibustion may be better or equivalent in clinical curative effects,but side effects of moxibustion is less; different moxibustion and acupuncture needle methods presented significant differences in curative effects; moxibustion plus acupuncture needle showed better curative effects. However,it is necessary for high quality trials to confirm its curative effects.
