MR三维磁化准备快速梯度回波技术分析脑转移瘤与瘤周血管的相关性 被引量:5

Analysis of correlation between intracranial metastasis and peri-tumor vessels using three-dimensional magnetization prepared rapid acquisition gradient echo sequences

摘要 目的采用低毒性钆布醇延迟增强三维磁化准备快速梯度回波(3D-MPRAGE)技术分析脑转移瘤与瘤周血管的相关性。方法回顾分析25例诊断为脑转移瘤的患者资料,采用3D-MPRAGE序列双倍剂量延迟增强扫描技术进行图像采集。分析脑转移瘤与不同分型瘤周血管的相关性,并进行两次图像评价的一致性检验。结果 25例患者共发现118个转移瘤病灶及573条瘤周血管,肿瘤大小与瘤周血管数呈正相关(r=0.75,P<0.05),Ⅰ、Ⅱ、Ⅲ型血管分别与肿瘤大小呈正相关(r=0.44、0.32、0.74,P均<0.05)。对病灶数目和血管分型的两次评价具有很好的一致性(病灶数目和Ⅰ型血管数:Kappa=1;Ⅱ、Ⅲ型血管数:Kappa=0.886、0.861)。结论增强3D-MPRAGE技术可显示脑转移瘤与瘤周血管的关系,瘤周血管可分为3型,其分型与病灶大小有一定的相关性。 Objective To investigate the correlation of intracranial metastasis and peri-tumor vessels using hypotoxic gadobutrol delayed contrast enhancement three-dimensional magnetization prepared rapid acquisition gradient echo sequences(3D MPRAGE).Methods Twenty-five patients with intracranial metastasis were retrospectively enrolled,who had received delayed double-dose gadobutrol contrast enhancement of 3D-MPRAGE.The relationship between intracranial metastasis and different peri-tumor vessels was analyzed,and consistency test was made between the two imaging evaluations.Results There were 118 metastatic lesions and 573peri-tumor vessels in the 25 patients,the tumor size and number of peri-tumoral vascular were positively correlated(r=0.75,P〈0.05).TypeⅠ,Ⅱ,Ⅲ of peri-tumor vessel were respectively positively correlated to the tumor size(r=0.44,0.32,0.74,all P〈0.05).The both evaluations of lesions number and peri-tumor vessel types had good consistency(number of lesions and peri-tumor vessel of TypeⅠ:Kappa=1;number of peri-tumor vessel of typeⅡ and Ⅲ:Kappa=0.886,0.861).Conclusion Delayed and double-dose gadobutrol contrast enhancement MR 3D-MPRAGE can clearly show the relationship between intracranial metastasis and peri-tumor vessel,which is classified as three types,and each type correlates with its lesions' size.
