摘要
目的探讨冠脉介入治疗对冠心病急性心肌梗死患者血浆B型钠利尿肽(BNP)水平和心室重塑的影响,为临床治疗方案的制定提供参考。方法选取冠心病急性心肌梗死患者78例,根据患者意愿将其分为介入治疗组38例和非介入治疗组40例。两组患者均分别于入院时、入院后第2天、第3天和第7天进行肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)和BNP水平的检测,同时在入院7~10 d内进行心脏彩色超声多普勒检查。对比两组患者的CK、CKMB峰值,不同时间的BNP水平和心脏彩色超声多普勒[(左心室收缩末期容积(ESV)、左心室舒张末期容积(EDV)、左室射血分数(LVEF)等指标)]检查结果。结果介入治疗组和非介入治疗组的CK峰值、CK-MB峰值对比,差异均无有统计学意义(P〉0.05)。介入治疗组ESV、EDV均显著低于非介入治疗组(均P〈0.05),而两组患者LVEF值的对比,差异无统计学意义(P〉0.05)。两组患者就诊时和入院第2天的BNP水平对比,差异无统计学意义(P〉0.05),介入治疗组入院第3天、第7天的BNP水平均显著低于非介入治疗组(均P〈0.05)。结论冠心病急性心肌梗死患者早期行冠脉介入治疗可减缓心室重塑,促进BNP水平的下降。
Objective To explore the effect of coronary artery interventional therapy on plasma level of B type natriuretic peptide( BNP)and left ventricular remodeling in patients with myocardial infarction in order to provide the reference for clinical treatment. Methods A total of78 patients with acute myocardial infarction in this hospital were selected for this study,38 of them were listed as intervention group and 40 in non- intervention group according to the wishes of patients. Patients in these 2 groups in second day,third day and seventh day after admission were examined for plasma levels of creatine kinase( CK),creatine kinase isoenzyme( CK- MB) and BNP,and cardiac color Doppler ultrasound imaging had been performed in 7 ~ 10 days after admission. The plasma level of CK and the peak value of CK- MB of patients in these two groups were compared,and BNP level at different time points and results of cardiac color Doppler ultrasound imaging( ESV,EDV,LVEF etc) were also compared between these 2 groups. Results The peak plasma levels of CK and CK- MB in patients of interventional treatment group and non interventional treatment group were compared,and the difference was not statistically significant( P〉0. 05). The figures of ESV and EDV in patients of interventional treatment group were significantly lower than those of patients in non interventional group( P〈0. 05). The difference in EF was not statistically significant between these two groups. In comparison with BNP levels in patients of these two groups at the day of admission and 2 days after admission,the difference was not statistically significant( P〉0. 05). The plasma levels of BNP in patients of interventional treatment group in third day and 7th day after admission were significantly lower than those of patients in non- interventional treatment group( P〈0. 05). Conclusion The early interventional therapy in patients with myocardial infarction can alleviate ventricular remodeling and promote the decrease in level of BNP.
