慢性阻塞性肺疾病患者细菌感染的细胞外基质重构机制研究 被引量:3

摘要 目的对慢性阻塞性肺疾病(COPD)患者细菌感染的细胞外基质重构机制进行研究,为临床研究提供参考。方法选取医院2010年5月-2014年5月诊治的390例COPD患者,将其中合并细菌感染患者200例设为感染组,无细菌感染患者190例设为COPD组,以同期健康体检者90名为对照组;鉴定感染患者的病原菌种类;酶联免疫吸附测定法检测患者血清中炎症因子水平以及细胞外基质金属蛋白酶(MMP)2、9和TIMP1/2表达变化;采用SPSS20.0软件对数据进行统计分析。结果 200例COPD合并细菌感染患者送检标本中共分离出病原菌205株,其中革兰阳性菌98株占47.8%,以肺炎链球菌和金黄色葡萄球菌为主,革兰阴性菌107株占52.2%,以肺炎克雷伯菌和铜绿假单胞菌为主;感染组患者表皮生长因子(EGF)、肿瘤坏死因子α(TNFɑ)及缝隙连接蛋白(Connexin40)和Connexin43水平均明显升高,MMP2、9、TIMP1和TIMP2水平明显上升,与对照组和COPD组比较,差异均有统计学意义(P<0.05)。结论 COPD患者细菌感染引起炎症因子水平的异常和细胞外基质的重构。 OBJECTIVE To study the extracellular matrix remodeling of bacterial infections in patients with chronic obstructive pulmonary disease(COPD)so as to provide guidance for clinical research.METHODS A total of 390 COPD patients who were treated in the hospitals from May 2010 to May 2014 were enrolled in the study;200patients complicated with bacterial infections were assigned as the infection group,and 190 patients without bacterial infections were set as the COPD group;totally 90 people who received the physical examination during the same period were chosen as the control group.The pathogens causing the infections were identified.The levels of serum vascular epidermal growth factor(EGF),tumor necrosis factorα(TNFɑ),Connexin 40,and Connexin 43 were determined by means of enzyme-linked immunosorbent assay,and the changes of expression of extracellular matrix metalloproteinase(MMP)2,9,and TIMP1/2were observed;the statistical analysis of data was performed with the use of SPSS20.0software.RESULTS Totally 205 strains of pathogens were isolated from the submitted specimens that were obtained from 200 COPD patients complicated with bacterial infections,including 98(47.8%)strains of gram-positive bacteria and 107(52.2%)strains of gram-negative bacteria;the Streptococcus pneumoniae and Staphylococcus aureus were the predominant species of gram-positive bacteria;the Klebsiella pneumoniae and Pseudomonas aeruginosa were dominant among the gram-negative bacteria.The levels of the serum EGF,TNFɑ,Connexin40,and Connexin43 of the patients with infections were remarkably elevated;the levels of MMP2,9,TIMP1,and TIMP2 were significantly elevated,as compared with the control group and the COPD group(P〈0.05).CONCLUSIONThe bacterial infections in the COPD patients may results in the abnormal levels of inflammatory factors and the extracellular matrix remodeling.
