摘要
目的探讨视频头脉冲试验(video head impulse test, vHIT)在评价前庭康复治疗前庭神经炎(vestibular neuritis, VN)疗效的应用。方法选取2017年6月至2018年6月于西安交通大学第一附属医院门诊诊断为VN的26例患者,分为对照组(12例)与前庭康复组(14例),比较患者治疗前、治疗1周后、治疗后1个月的vHIT结果,并进行眩晕残障程度评定量表(dizziness handicap inventory, DHI)评分,观察并评价前庭康复的疗效。结果两组治疗前vHIT显示受累半规管前庭眼动反射(vestibular-ocular reflex, VOR)增益值减小,相应不对称值增大,有显性扫视。两组治疗1周后增益值有所恢复,与治疗前无明显差异。1个月后对照组7例(58.33%)增益值恢复正常,前庭康复组12例(85.71%)增益值恢复正常,组间疗效比较差异有统计学意义(P <0.05)。治疗1周后和1个月后与治疗前相比,DHI评分均显著降低,前庭康复组的DHI评分均低于对照组,差异均有统计学意义(P <0.01)。结论 v HIT有助于VN的诊断与随访,并能有效评价前庭康复治疗的疗效。
Objective To evaluate the efficacy of vestibular rehabilitation for the patients with vestibular neuritis(VN by using video head impulse test(vHIT).Methods A total of 26 patients diagnosed with VN were enrolled in the outpatient department of the First Affiliated Hospital of xi’an Jiaotong University from June 2017 to June 2018.They were divided into the drug therapy group and the vestibular rehabilitation group.The dizziness handicap inventory(DHI)scale and the results of vHIT were used to evaluate the efficiency of vestibular rehabilitation.The results of before treatment,one week and one month of treatment were compared.Results Before treatment,vHIT of the two groups revealed vestibular-ocular reflex(VOR)gain decreased,and asymmetry and dominant saccadic vision increased.There was no significant difference between the two groups after 1-week treatment.After one month,7 cases(58.3%)of the control group and 12 cases(85.7%)of the vestibular rehabilitation group returned to normal gain values.The difference in efficacy between the two groups was statistically significant(P<0.05).DHI scores in the vestibular rehabilitation group were significantly lower than those in the control group after one week and one month of treatment(P<0.01).Conclusions vHIT is helpful for the diagnosis and follow-up of VN,which can effectively evaluate the curative effect of vestibular rehabilitation.
