摘要
目的研究伴中央颞区棘波的儿童良性癫痫(BECT)患儿经奥卡西平单药治疗6个月后其注意网络的变化。方法运用注意网络测试工具(ANT)对服用奥卡西平治疗6个月前后的BECT患儿进行注意网络测试。结果在奥卡西平治疗6个月过程中,76.1%的BECT患儿未有发作,13.0%的患儿发作次数较用药前减少≥50%。71.8%的患儿在奥卡西平治疗6个月后脑电图异常放电得到缓解。BECT患儿经奥卡西平治疗6个月后(T1阶段)注意网络总反应时(876.00±186.68)ms低于治疗前基线水平(T0阶段)(956.76±190.38)ms,差异有显著统计学意义(t=4.111,P=0.000)。T1阶段注意网络检查正确率(97.46±3.20)ms高于T0阶段(95.76±5.38)ms,差异有显著统计学意义(t=-2.578,P=0.013)。警觉网络功能效率比较,T1阶段73(40,99)ms高于T0阶段45(17,84)ms,差异有统计学意义(z=-2.125,P=0.034);定向网络效率T1阶段32(-6,71)ms高于T0阶段3(-27,57)ms,差异有统计学意义(z=-2.213,P=0.027)。治疗前后执行网络功能效率差异无明显统计学意义(P>0.05)。结论奥卡西平可改善BECT患儿的临床发作及脑电图异常放电并可改善患儿注意网络功能,特别是警觉网络功能。
Objective: To investigate the attention networks evaluation in children with benign epilepsy with centrotemporal spikes (BECT) treated by oxcarbazepine (OXC) therapy. Methods: Attention networks assessment was performed at baseline and after 6 months treatment with the efficiency of three anatomicaly defined attention networksalerting, orienting, and executive control. Results: 46 children were completed the research, During the follow-up, sustained cessation of seizures under medication was observed in 76.1% of patients; 13.0% experienced 〉/50% improvement, and 10.9% showed no improvement. Meanwhile, 71.8% showed an improvement in the grade of electroencephalographic pathology, and 24% manifested no change at all. There was no normalization of interictal epileptiform activity was observed. The Grand mean effect of attention network test was significantly lower (P 〈 0.05) in T1 (after 6 months of OXC therapy) (876.00+186.68) ms than that in T0 (baseline) (953.93±177.05) ms. The correct rate of attention network test was significantly higher (P 〈 0.055 in T1 (97.46+3.20) ms than that in TO (95.76+_5.38) ms. The alerting networks effects (73 (40, 99) ms) was higher in T1 than in TO (45 (17, 84) ms) with significant difference, The orienting networks effects (32 (-6, 71) ms) was higher in Tt than in TO (18 (-22, 56) ms) with significant difference, however, we found no significant difference in the executive control networks effects. Conclusion: The current study suggested that OXC may effective in preventing seizures and normalizing electroencephalograms and seems to preserve attention network functions as short-term monotherapy in children with typical BECTS.
