研究鲍曼不动杆菌外排泵在氟喹诺酮类药物耐药中的作用 被引量:3

摘要 目的研究外排泵基因ade B的存在与鲍曼不动杆菌对氟喹诺酮类药物耐药的相关性,以及外排泵抑制剂羰基氰化物间氯苯腙(CCCP)对耐药率的影响,以探讨主动外排作用在鲍曼不动杆菌耐药中所起的作用。方法采用K-B琼脂纸片扩散法进行药敏试验,琼脂稀释法检测环丙沙星、左氧氟沙星和加替沙星等3种氟喹诺酮类药物对58株鲍曼不动杆菌的MICs,同时检测在加入CCCP后3种药物MICs的变化情况。结果 58株鲍曼不动杆菌临床菌株中,45株至少对1种氟喹诺酮类药物耐药,13株对3种氟喹诺酮类药物全部敏感,总耐药率为77.6%(45/58)。加外排泵抑制剂CCCP后,鲍曼不动杆菌临床菌株对3种氟喹诺酮类药物的耐药率都降低:45株耐药株中31株外排泵表型阳性。对3种药物全部敏感的13株菌株中有1株外排泵表型阳性。鲍曼不动杆菌耐药株外排基因ade B检出率(100%)明显高于敏感株(76.9%)(χ~2=11.0,P〈0.01)。结论 CCCP在体外可以抑制鲍曼不动杆菌对氟喹诺酮类药物主动外排作用,降低鲍曼不动杆菌的MICs值。外排基因ade B的存在与鲍曼不动杆菌对氟喹诺酮类药物的耐药性具有相关性。 Objective To investigate the correlation of efflux pump gene ade B with the fluoroquinolones( FQNs) resistance in A. baumannii,and the effect of carbonyl cyanide- m- chlorophenylhydrazone( CCCP) on the resistance rate,so as to elucidate the role of active efflux in the resistance of A. baumannii. Methods The antibacterial agent susceptibility tests were performed by the disc diffusion method( K- B). The minimum inhibitory concentration( MICs) of each FQNs was tested by the agar dilution method,and the MICs changes of the 3 drugs were detected after adding CCCP. Results Among 58 isolates,there were45 isolates presenting at least for one kind of FQN resistance,13 isolates presenting susceptible to all FQNs,with the total resistance rate of 77. 6%( 45 /58). After CCCP was used,the resistance rate of A. baumannii to 3 FQN reduced. 31 isolates were positive in phenotype tests of efflux pump in 45 resistant isolates,and one isolate was positive in phenotype tests of efflux pump in 13 suspective isolates. The detection rate of ade B gene in resistant isolates( 100%) was significantly higher than in the sensitive isolates( 76. 9%)( χ2= 11. 0,P〈0. 01). Conclusion CCCP could inhibit the active efflux of A. baumannii to FQNs and reduce the MICs of Vitro. There was correlation between the existing of efflux pump gene ade B and FQNs drug resistance in A.baumannii.
