摘要
目的探讨超声在鉴别诊断甲状腺滤泡癌和滤泡性腺瘤中的价值。方法回顾性分析2010年3月至2015年3月于我院行手术且病理证实为甲状腺滤泡癌患者30例33个结节和甲状腺滤泡性腺瘤患者38例41个结节的超声特征。重点分析比较两类疾病结节的最大直径、形态、边界、声晕、实质回声强度、实质回声均匀性、囊性成分、钙化及声触诊组织量化(VTQ)值。结果甲状腺滤泡癌的声晕、回声强度、囊性成分、钙化及VTQ值与甲状腺滤泡性腺瘤间的差异具有统计学意义(P<0.05),其中实性回声、无声晕、低回声及钙化在甲状腺滤泡癌中更常见,而薄且均匀声晕、等回声、少量或中量囊性成分及少钙化则在甲状腺滤泡性腺瘤中更常见。根据部分结节剪切波传播速度(SWV)值构建受试者工作特征(ROC)曲线,当SWV值为1.84 m/s时,ROC曲线下面积最大,其敏感性和特异性分别为82.4%、73.9%。结论超声在诊断与鉴别诊断甲状腺滤泡癌和滤泡性腺瘤方面具有重要的临床价值。
Objective To explore the value of ultrasound features in differentiating between thyroid follicular carcinoma and follicular adenomas. Methods 33 pathologically proven thyroid follicular adenoma in 30 patients and 41 thyroid follicular carcinoma in 38 patients who underwent thyroid surgeries in our hospital during March 2010 to March 2015 were included in the retrospective study. US features of each nodule, including the maximum diameter, shape, boundary, presence of halo, echogenicity of parenchyma, the echoic homogeneity of the parenchyma, the component of the cystic the presence of calcification and the value of VTQ was analyzed. Results There were statistically significance in the presence of halo, echogenicity of parenchyma, the component, of the cystic the presence of calcification and the VTQ values between the ultrasound features of TFC and FA (P<0.05). Acoustic halos, the echo intensity, cystic component and calcification were more common in thyroid follicular carcinoma than thyroid follicular adenoma, while solid echo, silent dizzy, low echo, calcification were more common in thyroid follicular adenoma. Moreover, According to the ROC curve, a SWV cutoff value of greater than 1.84 m/s yielded sensitivity and specificity values of 82.4% and 73.9%, respectively, for diagnosis of FTC and FA. Conclusion Ultrasound is of great application value in the diagnosis and differential diagnosis of thyroid follicular carcinoma and follicular adenoma.
