摘要
目的观察归芪消癥汤合正清风痛宁缓释片治疗Ⅳ期糖尿病肾病(DN)的临床疗效。方法将42例Ⅳ期DN患者随机分为2组,对照组21例采用贝那普利治疗,治疗组21例采用归芪消癥汤合正清风痛宁治疗。2组均4周为1个疗程,连用2个疗程。比较2组临床疗效及治疗前后中医证候积分、24 h尿蛋白定量、血白蛋白、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A1c)及血清肌酐(Cr)水平变化。结果治疗组总有效率(76.19%)优于对照组(52.38%,P<0.05)。2组治疗后中医证候积分、24 h尿蛋白定量、FPG、2 h PG、Hb A1c均较本组治疗前降低(P<0.05),血白蛋白增加(P<0.05)。且2组中医证候积分、24 h尿蛋白定量及血白蛋白组间比较差异有统计学意义(P<0.05)。结论归芪消癥汤合正清风痛宁为治疗Ⅳ期DN的有效方法,作用明显优于西药。
Objective To observe the clinical effect of Guiqi - xiaozheng decoction combined with Zhengqing - fengtongning tablets on Ⅳ stage diabetic nephropathy (DN). Methods 42 Ⅳ stage DN patients were randomly divided into two groups. 21 patients in control group were treated by benazepril hydrochloride tablets. 21 patients in treatment group were treated by Guiqi - xiaozheng decoction combined with Zhengqing - fengtongning tablets. The treatment course was four weeks in two groups lasted two courses. The curative effect, traditional Chinese medicine (TCM) syndrome score, the levels of 24 h urine protein excretion, serum albumin, fasting blood glucose (FPG) , 2 h postprandial blood glucose (2 hPG) , hemoglobin A1C (HbA1C) and serum creatinine were observed in two groups. Results The total effective rate in treatment group (76. 19% ) was superior to that in control group ( 52.38% , P 〈 0.05 ). The TCM syndrome score, 24 h urine protein excretion, FPG, 2 hPG and HbA1C after treatment were decreased in two groups ( P 〈 0.05 ) , and the level of serum albumin was increased ( P 〈 0.05 ). There were statistical differences on TCM syndrome score, 24 h urine protein excretion and serum albumin between two groups ( P 〈 0.05 ). Conclusion Combined therapy of Guiqi - xiaozheng decoction and Zhengqing - fengtongning tablets has an exact effect on the treatment of Ⅳ stage diabetic nephropathy (DN) , and its effect is superior to western medicine.
