摘要
目的探析血栓弹力图(TEG)在心脏外科术后早期输血液制品中应用价值。方法选择我院心脏外科2013年4月~2014年10月收治76例择期行心脏外科手术患者作为研究对象,分别进行TEG及凝血功能检测,比较阳性率。结果输注前、输注200 ml、400 ml新鲜冰冻血浆(FFP)后3种不同状态下,R、K、Angle等指标间的两两比较均有统计学意义(P〈0.05),A不同状态下的两两比较均无统计学意义。TT、aPTT、Fbg指标输注前、输注200 ml、400 ml FFP后3种不同状态之间的两两比较均有统计学意义(P〈0.05),PT指标输注400 ml与输注200 ml及输注前的比较均有统计学意义(P〈0.05),但输注前与输注200 ml的比较无统计学意义。TEG检测阳性率为85.1%(194/228),高于凝血象检测73.7%,对比χ~2=9.059,P〈0.05,证实TEG敏感性更高。结论 TEG在心脏外科患者术后早期输血中有较高应用价值,可为术后输血情况提供有力指导,纠正并预防术后凝血障碍的同时避免血液资源浪费,安全简便,值得推广。
Objective To analyze the application value of thromboelastography( TEG) in early transfused blood products after cardiac surgery. Methods 76 patients receiving cardiac surgery in selected time in the hospital from April 2013 to October 2014 were selected as the research object. The TEG and blood coagulation were detected,respectively. And the positive rate was compared. Results In before infusion,after infusion of 200 ml,400 ml of FFP 3 different states,the two two comparison of R,K,Angle and other indicators was statistically significant( P〈0. 05) while the comparison of A under different conditions were not statistically significant. The index TT,aPTT and Fbg in before infusion,after infusion of 200 ml,400 ml of FFP 3 different states were two two compared and the differences were all statistically significant( P〈0. 05) and the same with index PT( P〈0. 05),but the comparison before infusion and transfusion of 200 ml was not statistically significant. The positive rate of TEG detection was 85. 1%( 194 /228) which was higher than 73. 7% of blood coagulation detection,compared with χ~2= 9. 059,P〈0. 05,confirming that TEG is more sensitive. Conclusion The TEG is of higher application value in early blood transfusion after cardiac surgery,can provide powerful guidance for blood transfusion after operation,avoid the waste of resources while correcting and preventing blood coagulation disorder,and is safe and convenient,worthy of promotion.
