摘要
目的探讨经直肠超声造影(CETRUS)参数峰值强度对前列腺周缘区良恶性结节的诊断价值。方法对48例经直肠超声检查发现的前列腺单侧周缘区结节患者进行超声造影检查,利用QLab软件测量每个结节的峰值强度(PIt)及对侧周缘区组织峰值强度(PIn),所有结节良恶性均通过经直肠超声引导下穿刺活检病理证实。结果经直肠超声发现周缘区结节49个,经直肠超声引导下穿刺活检证实恶性结节20个,良性结节29个;PIt鉴别良恶性结节敏感性55.0%,特异性69.0%,联合PIn后鉴别良恶性结节敏感性和特异性均显著提高,分别为81.3%、78.8%。结论 CETRUS参数PIt联合PIn对于鉴别良恶性结节较单一参数的特异性、敏感性显著提高,为临床诊断前列腺癌提供了更为理想的检测指标。
Objective To evaluate the diagnostic value of perfusion parameter peak intensity(PI)to predict prostate cancer occurred in peripheral zone(PZ) using transrectal contrast- enhanced ultrasound(TRCEUS). Methods Forty- eight patients with single-side abnormal PZ nodules found by transrectal ultrasound were enrolled in this study.Data of TRCEUS for each patient was retrospectively analyzed.The perfusion variables of peak intensity of abnormal nodules(PIt)and the contrast area on the other side(PIn) were measured using CEUS with software QLab.Compared with the pathologic results,the diagnostic value(sensitivity,specificity)to predict the prostate cancer using TRCEUS perfusion variable PI was calculated.Results In the total 49 prostate PZ nodules,20 were malignant,29 were benign.The sensitivity and specificity of using Plt to predict the prostate cancer are 55.0% and 69.0%,respectively,while combined with the Plt,the sensitivity and specificity were 81.3% and 78.8%,respectively.Conclusion Sensitivity and specificity using perfusion variable PIt combined with PIn are much higher than using the Plt only in differentiation diagnosis for the prostate cancer found by contrast-enhanced transrectal ultrasound in PZ.The PI,one of the perfusion variables,maybe a useful method in prostate cancer prediction in future.
