摘要
目的比较度洛西汀和米氮平治疗老年抑郁症同时主诉躯体疼痛或失眠的患者的疗效及安全性。方法选取主诉躯体疼痛或失眠的老年抑郁者患者各60例,随机分为度洛西汀治疗组和米氮平治疗组,治疗8周,采用汉密尔顿抑郁量表(HAMD)、临床疗效总评量表(CGI)和不良反应量表(TESS)记录并考察治疗效果及不良反应,同时对患者躯体疼痛和失眠症状进行评价。结果治疗8周后,在抗抑郁症疗效方面,两组治疗前后比较均有显著性差异(P<0.01),但两组间比较无显著性差异(P>0.05)。在不良反应方面,两组间TESS评分无显著性差异(P>0.05)。在缓解躯体疼痛症状方面,度洛西汀组有效率显著高于米氮平组(P<0.05);而在改善失眠症状方面,米氮平组的PSQI评分低于度洛西汀组(P<0.05)。结论度洛西汀和米氮平治疗老年抑郁症效果及不良反应相当,但度洛西汀缓解躯体疼痛症状更优,而米氮平改善失眠症状更优。
Objective To compare the efficacy and safety of duloxetine and mirtazapine in the treatment of senile depression with complaint of body pain or insomnia. Methods 60 patients with senile depression complaining of body pain or insomnia were selected,and randomly divided into duloxetine and mirtazapine treatment group with8 weeks therapy. Hamilton depression scale（ HAMD）,Clinical Global Impression Scale（ CGI） and Treatment Emergent Symptom Scale（ TESS） were used to record and inspect the treatment effect and adverse reactions. The body pain and insomnia of patients were also evaluated. Results After 8 weeks＇ treatment,in terms of antidepressant effect,comparisons of these two groups before and after the treatment are significantly different（ P〈0. 01）,while there was no significant difference between these two groups（ P〉0. 05）. In terms of adverse reactions,there saw no obvious difference of TESS between these two groups（ P〉0. 05）. As in terms of alleviating body pain symptoms,effective rate of the duloxetine group was significantly higher than that in the mirtazapine group（ P〈0. 05）; and as for improvement of insomnia,the PSQI of the mirtazapine group was lower than that in the duloxetine group（ P〈0. 05）.Conclusion The therapeutic effect and adverse reaction of duloxetine and mirtazapine in the treatment of senile depression are equivalent,but with the duloxetine much better in relieving body pain and mirtazapine in improving insomnia.
