摘要
目的 探讨自体筋膜移植再手术治疗腰椎术后粘连的效果。方法 对26例下腰椎术后粘连患者再手术,扩大椎板减压粘连松解加自体筋膜移植12例,后路内固定植骨加自体筋膜移植8例,椎间植骨融合加自体筋膜移植6例。结果 26例均获得随访,时间20~36个月。疗效依据中华骨科学会脊柱外科学组制定评定标准：优12例,良8例,差6例。结论 采用自体筋膜移植可以有效减少下腰椎术后粘连的再次发生。
Objective To explore the effect of reoperation for lumbar postoperative adhesion with transfer of fascial flap. Methods 26 patients with lumbar postoperative adhesion were treated by transfer of fascial flap,the expand laminectomy and adhesiolysis surgery and transfer of fascial flap were done for 12 cases,the posterior fixation and bone grafting and transfer of vascularized fascial flap were done for 8 cases,and intervertebral interbody fusion and transfer of vascularized fascial flap were done for 6 cases. Results 26 patients were followed up for 20 ~ 36 months,according to the assessment of spine society of spine surgery group,the results were excellent in 12 cases,good in 8 and poor in 6. Conclusion It is an effective method for the lumbar postoperative adhesion by reoperative treatment with fascial flap for preventing peridural adhesion.
