抗生素不同给药方式治疗呼吸机相关性肺炎的临床观察 被引量:6

摘要 目的对比抗生素静脉给药联合雾化吸入给药与抗生素单纯静脉给药治疗呼吸机相关性肺炎(VAP)的临床疗效及安全性。方法选取2012年6月至2014年6月收治的VAP患者60例,随机分为观察组与对照组,各30例。观察组采用抗生素静脉给药联合雾化吸入给药,对照组采用抗生素静脉给药,均以7 d为1个疗程。结果治疗后第7天及第14天观察组体温、白细胞计数和平均治疗时间较对照组明显改善(P<0.05);观察组2周痰培养转阴率及临床治愈率均显著高于对照组(P<0.05);两组患者复发率及病死率无统计学差异(P>0.05);治疗期间均无严重不良反应发生。结论采用抗生素静脉给药联合雾化吸入给药,患者症状消失快,临床治愈率高,可显著缩短治疗时间,安全性较好,是临床治疗VAP的有效手段。 Objective To investigate the clinical effect and safety of intravenous antibiotics combined with aerosol inhalation and intravenous antibiotics in the treatment of ventilator- associated pneumonia. Methods 60 patients from June 2012 to June 2014 were randomly divided into the observation group and the control group, 30 cases in each group. The observation group was treated with intravenous antibiotics combined with aerosol inhalation, while the control group was treated with intravenous antibiotics only. 7 d for1 course of treatment. Results The temperature and WBC number of the observation group were significantly reduced compared with the control group at 7 d and 14 d after treatment( P 〈 0. 05),and the mean treatment time of the observation group were shorter than the control group( P 〈 0. 05); the sputum culture negative rate and clinical cure rate were significantly increased compared with the control group after two weeks of treatment( P 〈 0. 05); the death rate and recurrence rate showed no statistical differences between two groups( P 〉 0. 05); no serious adverse reaction happened during treatment in two groups. Conclusion Patients treated with intravenous antibiotics combined with aerosol inhalation has a faster improvement of symptoms, a higher clinical cure rate, and a shorter treatment time with better security. It is effective in treatment of ventilator- associated pneumonia.
