摘要
目的:通过观察独活寄生汤加减内服并配合外洗治疗膝关节骨性关节炎的疗效,为进一步开展多中心、大样本、长期的随机对照临床实验提供依据。方法:将明确诊断为膝关节骨性关节炎的120例患者按数字表法随机抽样分为对照组和观察组各60例。对照组用西医方案治疗(口服双醋瑞因、盐酸氨基葡萄糖胶囊及关节腔注射玻璃酸钠),观察组使用独活寄生汤内服外洗的中医方案治疗,5周为1个疗程,观察治疗前后所有患者的临床证候疗效、膝关节功能评分(LKSS)、骨关节炎指数评分(WOMAC)及不良反应情况。结果:观察组有效率为88.33%,对照组有效率为71.67%,差异有统计学意义;治疗后,LKSS均高于治疗前观察组高于对照组;观察组与对照组的WOMAC总分和疼痛程度、僵硬程度、日常活动评分,治疗后均较治疗前有所改善,观察组明显优于对照组,2组相比较具有统计学意义;不良反应对照组明显高于治疗组。结论:独活寄生汤加减内服配合外洗可以有效治疗膝关节骨性关节炎,且具有较好的安全性,其具体的作用机制有待临床进一步研究。
Objective： By observing the internal and external washing alone live parasite soup treating knee joint os- teoarthritis 60 patients curative effect, in order to further carry out muhicenter, large sample and long-term randomized controlled clinical trials. Methods： a clear diagnosis of 120 patients of knee osteoarthritis by digital table method random sampling were divided into control group and observation group, 60 cases/group. Control group was treated with western medicine scheme （oral double vinegar, glucosamine hydrochloride capsule and joint cavity injection of sodium hyaluro- hate）. Observation group living by using internal and external parasitic soup wash solution of traditional Chinese medicine treatment, the five weeks as a course of treatment, all patients before and after treatment was observed the clinical syn- drome curative effect, the knee joint function score （LKSS）, osteoarthritis index score （WOMAC） and its adverse reactions. Results： observation group and control group total effective rate were 85% and 75% respectively （P〈0.05）; LKSS compari- son of two groups, end LKSS were higher than before treatment, the difference was statistically significant （P〈0.05）; LKSS treatment at the end of the observation group is higher than the control group, the difference was statistically significant （P〈0.05）. The WOMAC scores of observation group and control group, pain score, rigid degree score, daily activity score before and after treatment were improved, the observation group was better than control group, two groups are statistically significant （P〈0.05）; Adverse reactions observed group significantly lower than the control group.
