摘要
目的探讨实施综合管理及干预对提高社区痴呆患者照顾者防跌倒能力的作用。方法采用以街道为单位的整群随机化分组，将2个街道随机分为予以12个月的个体化综合管理干预的研究组和未进行管理的对照绀。根据抽样结果，共计186例社区居家痴呆患者照顾者参加并完成本次研究，其中研究组99例，对照组87例。采用自行设计问卷评价两组照顾者综合管理前后跌倒相关知识掌握、跌倒预防措施执行的变化及被照护的患者跌倒发生情况。结果管理12个月后，研究组照顾者的跌倒相关知识总分为（19．13±5．22）分，跌倒预防措施执行总分为（53．89±19．22）分，对照组分别为（13．18±6．43），（43．54±16．78）分，两组比较差芹有统计学意义（t值分别为3．47，3．89；P〈0．01）。研究期间，研究组照顾者照护的患者跌倒发生3例次，时照组患者跌倒发生15例次，两组比较差异有统计学意义（x^2=11．91，P〈0．01）。结论对居家痴呆患者照顾者实施综合干预管理，有助于提高照顾者防跌倒知识、技能并落实措施，减少被照护患者跌倒的发生，是确保和维护居家患者安全的可行与有效的管婵方法和途径。
Objective To discuss the effects of comprehensive management and intervention in improving the skills of fall prevention in community caregivers of patients with dementia. Methods Group randomization was conducted with sub-district as unit. The two sub-districts were randomly divided into intervention group and control group. The intervention group was applied the individualized comprehensive intervention management while the control group did not. A total of 186 caregivers had completed this research; among them, 99 caregivers behmged to the intervention group and 87 belonged to the control group. A self- designed questionnaire was conducted in both groups to evaluate the fall knowledge of caretakers before and alter the comprehensive intervention management, changes in the fall prevention interventions and incidents of fall indementia patients in both groups. Results After 12 months intervention, the fall-related knowledge score and the sore of implementation of prevention measures in fall in the intervention group was （ 19.13 ± 5.22） , （53.89 ± 19.22） ; the scores in the control group were （ 13.18 ± 6.43 ） and （ 43.54± 16.78 ） , the differences were statistically significant （t = 3.47,3.89 ; P 〈 0.01 ）. During the period of research, the fall incidence was 3 in the intervention group and 15 in the control group （ x^2 = 11.91, P 〈 0.01 ）. Conclusions Comprehensive intervention management has significant effects on improving the skills of fall prevention in family caregivers of patients with dementia. It is helpful in enhancing fall-prevention knowledge and skills, so as to effectively lower down the number of fall incidents of patients. It is a feasible and effective management method to ensure safety of homebound patients with dementia.
