摘要
目的研究牙科束缚治疗时低龄儿童及其家长的焦虑分布及不同告知方式对焦虑的缓解效果。方法样本人群分为对照组与干预组:对照组常规口头告知束缚治疗的优缺点,干预组附加采用书面方式,告知低龄儿童牙科治疗行为管理及牙科焦虑知识。研究首次和再次治疗时患儿及家长的焦虑分布;分析此焦虑与家长自身牙科焦虑及患儿口腔相关生活指数的相关性;分组分析后续治疗过程中患儿及家长的焦虑与患儿治疗配合的改善状况,初步研究不同告知方式的焦虑缓解效果。结果书面告知较常规口头告知对家长焦虑程度的降低有显著效果;束缚下牙科治疗患儿的焦虑与家长的焦虑及家长自身的牙科焦虑显著相关。结论低龄儿童束缚牙科治疗时,与家长的沟通效果至关重要,应提倡书面告知,丰富告知的内容和方式,严重焦虑患儿应选择全麻等非常规行为管理方式。
Objective To describe the family anxiety of young children receiving dental treatment under restraint and analyze the decreasing effect of different explaining style. Methods The children under restraint were allocated into experimental group and control group. The children of experimental group were given written explanation besides the oral explanation. The anxiety of the children and their parents were demonstrated and the correlation between the anxiety of children with the parents' anxiety, the parents' own das(dental anxiety scale) records and the OHRQL(oral health related quality of life) index were analyzed with SPSS software. The decreasing effect of written explanation was also analyzed. Results The anxiety of the parents in experimental group decreased significantly. The children's anxiety correlated with the anxiety of parents. Conclusion The communication effect and explanation style with parents was crucial to the decrease of young children's anxiety under restraint treatment. Try to explain in written form and increase the communication time maybe helpful. Referral to sedation treatment if needed.
