摘要
目的观察磷酸肌酸钠对一氧化碳中毒心肌损伤的保护作用。方法选择2012年1月—2014年6月收治的一氧化碳中毒患者100例,按照随机数字表法分为观察组(50例)和对照组(50例)。对照组患者采用常规治疗,观察组在常规治疗基础上加用磷酸肌酸钠治疗。检测2组患者治疗前后血清心肌肌钙蛋白(cTNI)、肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、N末端脑钠肽前体(NT-proBNP)水平变化,比较2组心电图改善情况治疗效果及不良反应发生率。结果治疗前,2组患者血清cTNI、MYO、CK-MB、NT-proBNP水平差异无统计学意义(t=0.162、0.039、-0.041、-0.028,P>0.05);治疗后,2组上述指标均显著下降,且观察组患者显著低于对照组(t=-2.974、-3.604、-3.051、-2.693,P<0.05)。观察组患者的治疗总有效率为96.0%,明显高于对照组的84.0%(x^2=4.000,P<0.05);观察组心电图改善有效率为94.0%,显著优于对照组80.0%(x^2=4.332,P<0.05)。结论磷酸肌酸钠治疗一氧化碳中毒能够有效降低血清cTNI、MYO、CK-MB、NT-proBNP水平,对心肌损伤保护较好,疗效显著,且无不良反应发生,值得临床推广。
Objective To observe the protective effect of sodium phosphocreatine on myocardial injury of carbon monoxide poisoning.Methods Selected from January 2012 to June 2014,100 cases of carbon monoxide poisoning patients,according to random digital table method,they were divided into observation group(50 cases) and control group(50 cases).In the control group,they were treated with conventional therapy,observation group also added sodium phosphate therapy.Before and after treatment,levels of cardiac troponin(cTNI),myoglobin(MYO),creatine kinase(CK-MB),N-terminal brain natriuretic peptide(NT-proBNP) were detected in both of the two groups,ECG improvement and incidence of adverse reactions were compared between the two groups.Results Before treatment,2 groups of patients' serum cTNI,MYO,CK-MB,NT-proBNP level showed no statistical significance differences(t =0.162,t = 0.039,t =- 0.041,t =- 0.028,P〈0.05);after the treatment,these indices of the two groups were significantly decreased,and patients in the observation group were significantly lower than the control group(t =-2.974,t =-3.604,t =-3.051,t =-2.693,P〈0.05).Observation group's total efficiency was 96.0%,which was significantly higher than that of the control group' s 84.0%(x-2 =4.000,P〈0.05),observation group's ECG improvement was 94.0%,which was significantly better than control group's80.0%(x-2=4.332,P〈0.05).Conclusion Sodium phosphocreatine in the treatment of carbon monoxide poisoning can effectively decrease the levels of serum cTNI,MYO,CK-MB and NT-proBNP,better protect of myocardium from injury,it showed a significant effect with no adverse reaction,it is worth of clinical promotion.
