摘要
目的观察慢性乙型肝炎单病种分层临床路径实施效果。方法以2014年1—11月入院的符合慢性乙型肝炎(轻、中、重度)诊断标准及纳入临床路径标准的病例为路径组,2013年同期同病种未纳入临床路径标准的病例为对照组;比较两组的均次住院费用、药占比、均次药费、平均住院天数、治愈好转率等医疗质量指标,并总结分析慢性病单病种分层临床路径实施经验。结果单病种分层临床路径实施组的均次住院费用、药占比、均次药费较对照组均有所降低,治愈好转率升高;慢性乙型肝炎(重度)实施路径组平均住院日较对照组缩短2.33天。结论慢性病实施单病种分层临床路径管理可以有效控制医疗费用、提高医疗效率。
Objective To observe the effect of implementing clinical pathway （CP） in different degrees and single disease for Chronic Hepatitis B （CHB）. Methods All the cases, which were in accordance with the diagnos- tic criteria of CHB （mild, moderate and severe） and the inclusive criteria of CP from January to November 2014, were included in the pathway group. The cases of non - clinical pathway implementation, which were the same dis- ease during the corresponding period of 2013, were selected as the control group. Observations were carried out on the medical quality indexes of the two groups, including the average hospitalization expenses, the proportion of the medication, the average medication cost, the average hospitalization days and the healing rate. And the experience of CP is summarized and analyzed. Results Compared with the control group, the average hospitalization expenses, the proportion of the medication and the average medication cost were decreased and the healing rate was improved in the pathway group. In terms of the average hospitalization days, the severe type of CHB in the CP group were （2.33） d shorter than that of the control group. Conclusion The implementation of CP management in different degrees and single disease for chronic disease could effectively control the medical cost and improve the medical efficiency.
