超声弹性成像在甲状腺微小癌诊断中的应用价值 被引量:3

摘要 目的:探讨超声弹性成像在甲状腺微小癌诊断中的应用价值。方法:筛选笔者所在医院2013年1月-2015年6月收治的52例甲状腺微小癌患者作为研究对象。所有患者均经穿刺病理活检证实为甲状腺微小癌,对所有患者进行常规超声及超声弹性成像检查,对比常规超声与超声弹性成像对甲状腺微小癌的诊断结果,并分析甲状腺微小癌的超声弹性影像特点。结果:病理试验证实52例患者共69处病灶,常规多普勒超声确诊40例50处病灶,超声弹性成像确诊50例64处病灶,弹性成像诊断结果与病理证实结果比较无显著差异;但检出率明显高于常规超声,比较差异有统计学意义(P<0.05)。根据评价标准对不同病理类型的甲状腺微小癌的弹性超声进行分级评价,其评分结果比较差异有统计学意义(P<0.05)。结论:超声弹性成像对甲状腺微小癌的敏感性较高,可提高甲状腺微小癌的临床检出率,并且无创性检查安全性高,利于多次重复检查,具有临床应用及推广价值。 Objective: To investigate the value of ultrasonic elasticity imaging on diagnosis of thyroid cancer.Method: 52 patients with thyroid cancer were selected as the research object from January 2013 to June 2015 in our hospital.All patients were confirmed by needle biopsy of thyroid micro carcinoma and received conventional ultrasound and contrast-enhanced uhrasmmd elasticity imaging, the diagnosis result of conventional ultrasound and ultrasound elasticity imaging on diagnosis of thyroid micro carcinoma were compared and analyzed the features of thyroid micro carcinoma on ultrasound elasticity imaging.Result: Pathology experiment proved that there were 69 lesions in 52 patients, conventional doppler echocardiography confirmed there were 50 lesions in 40 patients, ultrasound elasticity imaging proved there were 64 lesions in 50 patients, the results of elasticity imaging diagnosis and pathological showed no significant differences, but the positive rate was significantly higher than conventional uhrasound, there was a statistical significancc(P〈0.05).According to the evaluation criteria of different pathological types of classification of elastic ultrasonic evaluation of thyroid micro carcinoma, the grading result comparison difference was statistically significant(P〈0.05).Conclusion: Ultrasound elasticity imaging has high sensitivity on detecting thyroid micro carcinoma, it can improve detection rate of clinical thyroid micro carcinoma and has high safety, which is beneficial to repeatedly checking and with clinical application and popularization value.
