实时荧光核酸恒温扩增技术在淋球菌检测中的应用评价 被引量:11

Evaluation on the application of real-time fluorescent isothermal nucleic acid amplification technology in the detection of Gonococcus

摘要 目的评价实时核酸恒温扩增检测技术(SAT)在淋球菌检测中的应用价值。方法对204例疑似淋球菌感染患者的生殖道拭子分别进行SAT、PCR法及培养法检测,同时对其尿液标本进行SAT检测。分别对生殖道拭子和尿液标本SAT检测结果与生殖道拭子PCR法和培养法检测结果进行比较分析。结果 204例疑似患者拭子标本中,SAT法检出阳性90例,高于培养法而略低于PCR法,但阳性率差异均无统计学意义(P>0.05)。其中有3例患者拭子标本SAT检测阳性而尿液标本检测为阴性,符合率为98.53%,两者阳性率差异无统计学意义(P>0.05)。以淋球菌培养为金标准,SAT检测拭子标本的敏感度为100%,特异度为91.94%;SAT检测尿液的敏感度为98.77%,特异度为94.31%。结论SAT技术检测拭子及尿液标本中的淋球菌与生殖道拭子PCR法和培养法效果相当,且尿液标本SAT检测作为一种非侵入性采样方法有望替代生殖道拭子标本检测。 Objective To evaluate the application of real - time fluorescent isothermal nucleic acid amplification technology in the detection of Gonococcus. Methods The genital swabs of 204 patients suspected with infection of Gonococcus were detected by SAT, PCR and culture. The urine specimens were also detected by SAT. The detection results of genital swabs and urine specimens with SAT were compared with that of genital swabs with PCR and culture. Results Of 204 genital swabs, 90 cases were detected by SAT, which was higher than culture but lower than PCR, and the differences on positive rates were all not sta- tistically significant(P 〉0.05 ). There were 3 cases of patients whose swabs test was positive, but urine samples test was nega- tive, with the coincidence rate of 98.53% , and the difference of the positive rates was not statistically significant( P 〉 0.05 ). Regarding Gonococcus culture as gold standard, the sensitivity of SAT test on swabs was 100%, specificity was 91.94%, the sensitivity of that on urine was 98.77%, specificity was 94.31%. Conclusion SAT test has a equivalent efficiency in the de- tection of Gonococcus in swabs and urine with PCR and culture, and as a non - invasive way of specimen collection, which might replace genital swabs test.
