摘要
目的研究佛山市南海区实施家庭医生式服务试点工作后,社区卫生服务中心对高血压患者的管理效果。方法通过南海区健康信息平台系统的健康档案资料,选取与社区服务中心签约家庭医生式服务协议的高血压患者120例,随机分成干预组和对照组各60例。对照组采用普通门诊治疗;干预组在普通门诊治疗的基础上由家庭医生服务团队为其提供家庭医生式服务,为患者提供健康评估、健康干预并追踪健康管理项目,为期12个月,研究实施家庭医生式服务前后高血压患者的管理效果。结果实施家庭医生服务后,干预组的血压控制率、高血压知识知晓率、自我管理能力、治疗依从性以及对社区卫生服务的满意度均高于对照组,差异均有统计学意义(均P<0.05)。结论家庭医生式服务对高血压患者的管理有显著的效果,可继续增加样本数量、观察时间及观察项目进行追踪研究。
[Objective]To investigation the health management effect of hypertension patients in community health service center after the implementation of service based on family doctor in Nanhai District of Foshan City.[Methods]Through the health record data from the health information platform system in Nanhai District, 120 hypertension patients who have signed the agreement of service based on family doctor with the community health service center were randomly divided into the intervention group and the control group, 60 cases in each group. The control group received the ordinary outpatient treatment, while the intervention group was given the family doctor service on the basis of ordinary outpatient treatment, which the contents included health assessment, health intervention and follow-up of health management items, and the duration was 12 months. The health management effect of hypertension patients before and after intervention was evaluated.[Results]After the implementation of the family doctor service, the blood pressure control rate, awareness rate of hypertension knowledge, self management ability,treatment compliance, and satisfaction toward community health service in the intervention group were higher than those in the control group, and the differences were statistically significant（all P〈0.05）.[Conclusion]The service based on family doctor has the significant effect on health management of patients with hypertension. It is necessary to increase the sample size, observation time and observation items to carry out the further research.
