肿瘤深度对超声弹性成像诊断乳腺肿物良恶性的影响

Impact of lesion depth on ultrasound elasticity imaging in diagnosis of malignant and benign breast tumor

摘要 目的:探讨肿瘤深度对超声弹性成像(UE)判断乳腺肿物良恶性的影响。方法:选择671例因乳腺肿块行手术切除患者作为对象,病灶792个。根据肿瘤深度的不同将其分为4组。对所有患者术前均行常规超声和超声弹性成像诊断,分别比较不同深度肿瘤UE成像效果、诊断准确度、灵敏度、特异度、阳性和阴性预测值,并比较良性与恶性乳房肿瘤的UE检查阳性率。结果:当肿瘤深度较浅时UE成像效果较好,当肿瘤深度>1.5 cm时UE成像效果较差;不同深度肿瘤组UE诊断准确度和特异度组间比较差异有统计学意义(P<0.05);随着肿瘤深度的增加,工作特征曲线(ROC曲线)及曲线下面积(AUC)不断降低。结论:肿瘤深度会影响超声弹性成像对乳腺肿物良恶性诊断的特异度与准确度。 Objective: To investigate impact of lesion depth on ultrasound elasticity imaging in diagnosis of malignant and benign breast tumor. Methods: Six hundred and seventy-one patients with breast lumps( 792 lesions)were enrolled in this study. They were divided into four groups according to the depth of lesion. All patients were tested with routine ultrasound diagnostic and ultrasound elasticity imaging. The imaging,diagnostic accuracy,sensitivity,specificity,positive and negative predictive values of different depth of lesion were compared. The lesion positive rate of diagnosis by UE was compared among same pathological type. Results: When depth of lesion was lower,the effect of UE was better,but lesion depth 〉1. 5 cm,it was worse; there was significant difference of diagnostic accuracy and specificity by UE in different depth of lesion( P〈0. 05); AUC of ROC curve decreased when the depth of lesion increased. Conclusion: The depth of lesion has impact on diagnostic specificity and accuracy of breast ultrasonicelastography.
