急诊气管插管呼吸机相关性肺炎患者感染病原菌分析 被引量:9

Pathogen characteristics and countermeasures of ventilator-associated pneumonia in patients received tracheal intubation in emergency department

摘要 目的分析急诊气管插管呼吸机相关性肺炎患者感染的病原菌分布及耐药率,为临床治疗提供参考依据。方法选取2013年1月-2015年1月医院因急诊气管插管导致呼吸机相关性肺炎患者64例为研究对象,采集患者痰液标本进行病原菌的鉴定及药敏试验,数据采用SPSS 18.0软件进行统计分析。结果 64例感染患者共检出病原菌102株,以革兰阴性菌为主,共70株占68.63%,其中鲍氏不动杆菌居于首位占38.24%;鲍氏不动杆菌对各类抗菌药物具有不同程度的耐药性,对头孢唑林的抗药性相对较高,达79.49%;铜绿假单胞菌对氨苄西林的耐药性达100.00%,肺炎克雷伯菌对氨苄西林和庆大霉素耐药率均为100.00%。结论应依据病原菌种类和药敏试验,选择合适的抗菌药物,以降低急诊气管插管导致呼吸机相关性肺炎患者感染病原菌的风险。 OBJECTIVE To analyze the distribution and drug resistance rate of pathogens infected by patients with ventilator-associated pneumonia caused by emergency intubation so as to provide references for clinical treatments, METHODS Totally 64 ventilator-associated pneumonia patients caused by emergency intubation in our hospital were selected as study objects. The sputum specimens of the patients were collected for pathogen identification and drug sensitivity test. All data were statistically analyzed by SPSS 18.0 software. RESULTS In 64 patients infected with pathogens, totally 102 strains of pathogens were detected, mainly accounting by gram-negative bacteria with 70 strains (68.63%). The proportion of Acinetobacter baumannii was in the primacy (38.24%). A. baumannii were resistant to various antimicrobial agents in different degrees and proved to be relatively highly resistant to ce- fazolin, reaching 79.49%. The resistance ratio of Pseudomonas aeruginosa to ampicillin was up to 100. 00%. Klebsiella pneumoniae was completely resistant to ampicillin and gentamicin. CONCLUSION Antibiotics should be chosen properly in accordance with the types of pathogens and the results of drug sensitivity test so that the infec- tion rate of ventilator-associated pneumonia in patients received emergency intubation can be reduced.
