乳腺癌改良根治术后调强放疗致放射性肺损伤的剂量学研究 被引量:14

摘要 目的探讨影响乳腺癌改良根治术后适形调强放疗致放射性肺损伤的剂量学因素及其预测价值。方法收集行乳腺癌术后适形调强放疗的57例女性患者,对其肺相关的剂量学因素进行单因素及多因素分析。结果 57例患者中共有7例发生放射性肺损伤,发生率为12.3%(7/57)。单因素分析结果显示患肺平均剂量(t=-4.764,P<0.05)、患肺最大肺剂量(t=-2.371,P<0.05)、健侧肺最大剂量(t=-2.607,P<0.05)、患肺V5(t=-5.227,P<0.05)、患肺V10(t=-5.120,P<0.05)、患肺V15(t=-4.711,P<0.05)、患肺V20(t=-4.229,P<0.05)、患肺V25(t=-3.835,P<0.05)、患肺V30(t=-3.579,P<0.05)、患肺V35(t=-3.719,P<0.05)、患肺V40(t=-3.820,P<0.05)、患肺V45(t=-3.578,P<0.05)与放射性肺损伤的发生有关。多因素分析结果显示V5是判断放射性肺损伤的独立预测因素,与放射性肺损伤关系最为密切。以V5=59.98%为诊断界值点,对放射性肺损伤的预测灵敏度为85.7%,特异度为96.5%。结论 V5对于乳腺癌患者改良根治术后行适形调强放疗后发生放射性肺损伤具有较好的预测价值,是独立的预后因素。
