摘要
目的探讨过敏性紫癜患儿发生肾损害的易感因素及相关免疫因素。方法选取2013年1月至2014年9月于本院住院治疗的150例过敏性紫癜患儿为研究对象,根据患儿是否发生肾损害分为肾损害组(71例)和非肾损害组(79例),比较两组患儿外周血T淋巴细胞亚群、胱抑素C(Cys-c)、血清免疫球蛋白(Ig A、Ig G、Ig M)、白三烯B4(LTB4)水平,并采用单因素法分析过敏性紫癜患儿发生肾损害的易感因素。结果肾损害组患儿CD3^+、CD4^+水平及CD4^+/CD8^+比值均显著低于非肾损害组(P<0.05),CD8^+水平显著高于非肾损害组(P<0.05)。肾损害组患儿Cys-c、Ig A、LTB4、肿瘤坏死因子-α(TNF-α)水平均显著高于非肾损害组(P<0.05);两组患儿Ig G、Ig M、IL-21水平比较差异均无显著性(P>0.05)。皮疹反复发生>1个月(OR=2.327)是过敏性紫癜患儿发生肾损害的易感因素。结论发生肾损害与非肾损害的过敏性紫癜患儿T淋巴细胞亚群、Cys-c、Ig A、LTB4、TNF-α等存在显著差异,单因素分析显示皮疹反复发生>1个月是并发肾损害的易感因素。
Objective To explore the susceptibility and immune factors of Henoch-Schonlein purpura children with renal damage.Method From January 2013 to September 2014,150 children with HenochSchonlein purpura in our hospital were selected as the research objects,and they were divided into renal damage group（71 cases） and non-renal damage group（79 cases）.Levels of T lymphocyte subpopulation,Cys-c,Ig A,Ig G,Ig M,LTB4 between the two groups were compared.The susceptible factors of renal damage in children with Henoch-Schonlein purpura were analyzed by single factor method.Result Levels of CD3^＋,CD4^＋ and CD4^＋/CD8^＋ ratio in renal damage group were significantly lower than non-renal damage group（P〈0.05）,CD8^＋ level was significantly higher than non-renal damage group（P〈0.05）.Levels of Cys-c,Ig A,LTB4,TNF-α in renal damage group were significantly higher than non-renal damage group（P〈0.05）;and levels of Ig G,Ig M,IL-21 between the two groups were not significantly different（P〉0.05）.The rash occurred repeatedly for more than 1 month（OR = 2.327） was susceptible factor to Henoch-Schonlein purpura children with renal damage.Conclusion There is significant differences T lymphocyte subsets,Cys-c,Ig A,LTB4,TNF-α of Henoch-Schonlein purpura children with or without renal damage.Single factor analysis shows that the rash occurred repeatedly for more than 1 month（OR = 2.327） is susceptible factor to HenochSchonlein purpura children with renal damage.
