摘要
目的探讨甲氨蝶呤联合超声引导下清宫治疗瘢痕妊娠的方法及临床疗效。方法选取剖宫产瘢痕妊娠5-9周共72例患者为研究对象,采用甲氨蝶呤注射后,血β-HCG下降,彩超下血供减少后,采用刮匙平行刮出妊娠组织,总结其治疗效果。结果 72例患者经治疗7-15 d后,血β-HCG水平低于治疗前,包块明显小于治疗前,有统计学意义（P〈0.05）。对于B超复查显示包块周围无血流信号或少许血流信号的66例患者行清宫术治疗,手术时间（6.5±1.5）min,出血量为（110.5±45.5）mL,治疗成功率为92.6%。6例患者采用局部病灶切除术加修补术。结论甲氨蝶呤联合超声引导下清宫治疗瘢痕妊娠的疗效满意。
Objective to investigate the method and clinical curative effect of methotrexate combined with ultrasound guidance in the treatment of scar pregnancy. Methods 72 cases of cesarean scar pregnancy patients with 5 -9 weeks were chosen as the research object, they were given methotrexate injection, and then were given parallel scraping curette pregnancy tissue after blood-HCG decreased and color Doppler ultrasound blood supply decrease. Their therapeutic effect was summarized. Results after 7d -15d of treatment, serum-HCG level in 72 cases of patients was lower than before, bag piece was smaller than before, there were statistically significant（P〈0.05）. 66 cases of patients that their B ultrasound examination showed that mass around without blood flow signal or a signal of blood flow were given uterine curettage treatment, the operation time was（6.5±1.5）min, the amount of bleeding was（110.5± 45.5）mL, the success rate of treatment was 92.6%. 6 cases were treated by local resection plus repair. Conclusion methotrexate combined with ultrasound guided curettage for the treatment of scar pregnancy curative effect is satisfactory.
