摘要
目的:观察中西医结合治疗糖尿病周围神经病变的临床疗效。方法:选取120例糖尿病周围神经病变患者,按照随机数字表法随机分为观察组和对照组各60例,对照组给予甲钴胺注射液和注射用鼠神经生长因子治疗,观察组在对照组治疗基础上给予黄芪桂枝五物汤治疗。2组均以30天为1疗程,治疗2疗程。观察2组治疗前后的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)、多伦多临床评分系统(TCSS)评分,比较2组的临床疗效。结果:治疗1疗程后,观察组总有效率95.00%,显著高于对照组的80.00%,差异有统计学意义(P<0.05)。治疗后,2组正中神经及腓总神经的MNCV及SNCV均较治疗前改善(P<0.05,P<0.01),观察组改善较对照组更为明显(P<0.05);2组TCSS评分均较治疗前下降(P<0.01),观察组下降较对照组更为明显(P<0.01)。结论:中西医结合治疗糖尿病周围神经病变疗效确切,能有效改善患者的临床症状和体征,值得在临床推广使用。
Objective： To discuss the curative effect of integrative Chinese and western medicine therapy for diabetic peripheral neuropathy（DPN）. Methods. One-hundred and twenty cases of patients with diabetic peripheral neuropathy were selected and randomly divided into the observation group and the control group , 60 cases in each group, according to the random number method. The control group was given Mouse nerve growth factor and cobalt amine, and the observation group was given Huangqi Guizhi Wuwu tang. All the two groups were treated for 60 days. Before and after treatment motor nerve conduction velocity（MNCV） and sensory nerve conduction velocityCSNCV）, Toronto clinical scoring system（TCSS） score of thetwo groups were recorded and compared. The clinical curative effect of the two groups was observed. Results. The total effective rate was 95.00% in the observation group, and was 72.9% in the control group, the difference being significant（P〈 0.05）. After treatment, MNCV and SNCV of median nerve and sural total nerve in the two groups were significantly improved（P 〈 0.05 or P 〈 0.01 compared with before treatment）, and the improvement in the observation group being superior to that in the control group（P 〈 0.05）. After treatment, TCSS scores in the two groups were all decreased significantly（P 〈 0.05）, and the decrease of the observation group being much more obvious than that in the control group（P 〈 0.05）. Conclusions. Integrated Chinese and western medicine therapy for DPN has reliable curative effect, and can effectively improve the clinical symptoms to vigorously promote clinic use.
