摘要
目的总结脊髓栓系松解术治疗成人脊髓栓系综合征的临床疗效。方法成人脊髓栓系综合征患者61例接受了脊髓栓系松解术。其中,终丝紧张型脊髓栓系综合征29例(A组),脂肪瘤型脊髓栓系综合征32例(B组),比较两组疗效。结果两组术后肢体疼痛和麻木均完全或部分缓解,肌力和膀胱功能明显改善。术后1个月,两组患者症状改善情况无统计学差异(P>0.05);术后1年,A组疼痛缓解和下肢肌力改善优于B组(P<0.05)。结论脊髓栓系松解术对成人终丝紧张型和脂肪瘤型脊髓栓系综合征均有确切疗效;在疼痛缓解和下肢肌力改善的远期效果方面,栓系松解术对终丝紧张型综合征的效果优于脂肪瘤型。
Objective To summarize the outcomes of surgical lysis for adult tethered cord syndrome（TCS）.Methods The surgical lysis for adult TCS was performed in 29 cases with tightened filum terminale type（group A）and 32 cases with lipoma type（group B）.The efficacy was cempared.Results The symptoms of pain and numbness of the legs were attenuated completely or partially and the power of muscles and bladder function were improved remarkably after operation in both groups.There was no significant difference in the improvements of the symptoms of TCS between two groups in one month after operations（P〉0.05）.While the improvements of pain and muscular power one year after surgery were more in group A than those in group B（P〈0.05）.Conclusion The efficacy of surgical lysis for adult TCS is definitly effective.In long-term efficacy,surgical lysis produces better outcomes in tightened filum terminale type of TCS than in lipoma type.
