摘要 目的探讨西门子128颞骨高分辨率CT(HRCT)在中耳疾病诊断中的应用价值。方法整群筛选2013年1月—2014年12月于该院就诊的中耳疾病患者86例,作为研究对象。其中慢性化脓性中耳炎62例,中耳畸形13例,颞骨骨折面瘫11例,采用HRCT对所有患者进行CT扫描,采用最大密度投影(MIP)、三维重建(SSD)等处理模式,观察所有患者HRCT影像结果并分析其特异性表现。结果中耳炎患者中HRCT确诊55例,确诊率达88.71%,中耳畸形者确诊12例,确诊率达92.31%,颞骨骨折致面瘫确诊率达100.00%;对比影像结果与术中所见可知HRCT显像较好,但对于面神经管、迷路及镫骨成像较差。结论 HRCT能够提高中耳疾病的临床确诊率,同时其对中耳内部微细结构的成像能够为中耳疾病显微手术提供指导,具有临床应用及推广价值。 Objective To investigate the application value of Siemens 128 temporal bone high-resolution CT (HRCT) in the diag-nosis of middle ear diseases. Methods 86 patients admitted in our hospital from January 2013 to December 2014 were selected as the research object, 62 of which were with suppurative otitis media, 13 with middle ear malformation, 11 with facial paralysis fol-lowing temporal bone fracture. HRCT CT scans were performed for all patients ,and the results and specificity of HRCT image of all the patients were observed by using MIP and SSD method. Results 55 patients with uppurative otitis media were confirmed by HRCT, and the diagnosis rate was 88.71%; 12 patients with middle ear malformation were confirmed by HRCT, and the diagnosis rate was 92.31%; the diagnosis rate of facial paralysis following temporal bone fracture was 100.00%. Imaging results and surgical findings showed that HRCT imaging was better, except for facial nerve canal, the labyrinth and the stirrup bone. Conclusion HRCT can improve the clinical diagnosis rate of middle ear disease, at the same time, by which the accurate imaging of middle ear can provide guidance for the micro operation for middle ear disease. Therefore it is worthy of clinical application and promotion.
