共焦显微镜下不同程度干眼角膜内树突状细胞观察研究 被引量:3

摘要 目的:比较不同程度干眼患者共焦显微镜下角膜内树突状细胞的密度、活化细胞比例,以期进一步揭示干眼的炎症机制,为干眼的临床诊断和治疗提供依据。方法:选择临床诊断为轻度、中度及重度干眼患者各20例,对其双眼进行共焦显微镜检查,对每幅共焦图片内角膜上皮下的树突状细胞进行计数,并换算成细胞密度(个/mm2)。比较不同程度干眼患者的树突状细胞密度的平均值。同时对不同程度干眼共焦显微镜下活化的树突状细胞比例进行统计比较。结果:轻度干眼上皮下树突状细胞密度为26.7±8.8个/mm2,其中活化细胞比率29.0±3.7%;中度干眼树突状细胞密度为69.6±11.1个/mm2,其中活化细胞比率83.5±7.6%;重度干眼树突状细胞密度为212.2±31.0个/mm2,其中活化细胞比率97.8±1.9%。干眼患者随其严重程度增加,角膜上皮下树突状细胞密度亦呈增加趋势,并且其上皮下活化的树突状细胞比例亦增加,具有统计学差异。结论:共焦显微镜下角膜内树突状细胞是干眼炎症的重要标志之一,其密度的变化和活化细胞的比例可作为评估干眼炎症程度的有效指标。 Objective:To objective the density of dendritic cellsand the proportion of activated cells of different degree of dry eye patients in vivo confocal microscopy,so as to investigate the mechanism of dry eye,to provide the basis for clinical diagnosis and treatment of dry eye.Methods:A case series with 60 consecutive cases of different degrees dry eye patients was investigated,examined with confocal microscopy in vivo,and at the same time the density of dendritic cells and the proportion of activated cells was recorded and analyzed with SPSS17.0.Results:In mild dry eye,dendritic cell density was 26.7±8.8/mm2,with activated cell 29.0±3.7%;In moderate dry eye dendritic cell density was 69.6 ± 11.1/mm2,with activate cell 83.5±7.6%;In severe dry eye dendritic cell density was 212.2±31.0/mm2,with activate cell 97.8±1.9%.The data showed that the corneal dendritic cell densityand the proportion of activated cells were found increased in more severe dry eye,with statistically significant difference.Conclusion:In vivo confocal microscope,dendritic cells is an important sign of dry eye inflammation.The proportion of activated cells and its density can be used as effective indicators to assess the degree of dry eye.
