4种新方法快速检测结核分枝杆菌的临床研究 被引量:12

A comparative study on the clinical application of 4 new rapid detection methods for Mycobacterium tuberculosis

摘要 目的评价4种新检测法对肺结核病的诊断价值。方法用Xpert Mtb/RIF、LED法、MGIT液体培养和LAMP试验对结核分枝杆菌进行检测,并与罗氏培养、萋尼染色结果比较。结果 MGIT液体培养检测阳性结果,平均时间为12.5 d,阳性率为46.0%;罗氏培养检测阳性结果,平均时间为26.4 d,阳性率为43.0%。LAMP试验检测MTB的阳性率与萋尼氏染色法、LED法、MGIT液体培养和罗氏培养法比较,差异有统计学意义(χ2值分别为45.1、35.3、24.7、30.8,P<0.01)。萋尼氏染色法阳性率与荧光染色LED法比较,差异无统计学意义(χ2=0.67,P>0.05)。结论 MGIT液体培养、LED法及LAMP试验检测结核分枝杆菌具有快速、特异等优点,有很好的临床应用和推广价值。 Objective To evaluate the diagnostic value of 4 kinds of new assays for Mycobacterium tuberculosis. Methods Xpert Mtb / RIF,light- emitting diode( LED),MGIT liquid culture and DNA loop- mediated isothermal amplification( LAMP) were used to detect Mycobacterium tuberculosis,and the results were compared with Lownstein- Jenson culture result and Zieh-Neelsen staining result. Results MGIT liquid culture showed the average time of 12. 5 days and the positive rate of 46. 0%;Lownstein- Jenson culture showed the average time of 26. 4 days and positive rate of 43. 0%; LAMP showed that the positive rates differences of MTB,Zieh- Neelsen staining,LED method,MGIT liquid culture and Roche culture were statistically significant( χ2= 45. 1,35. 3,24. 7,30. 8,P〈 0. 01). And there was no statistical significance on the difference between Zieh-Neelsen staining and fluorescence staining( χ2= 0. 67,P〈 0. 05). Conclusion MGIT liquid culture,LED assay and LAMP for detection of Mycobacterium tuberculosis has the advantages of rapidity,specification,and they have good clinical application and promotion value.
