摘要
目的探讨α-平滑肌肌动蛋白(ɑ-smooth muscle actin,α-SMA)、成纤维细胞活化蛋白(fibroblast-activating protein,FAP)在正常子宫颈、子宫颈上皮内瘤变(cervical intraepithelial neoplasias,CIN)、子宫颈浸润性癌组织的表达,并分析二者表达与子宫颈浸润性癌临床病理指标、浸润转移及预后的关系。方法应用免疫组化Eli Vision两步法检测20例正常子宫颈、20例CIN及98例子宫颈浸润性癌组织中α-SMA和FAP的表达。比较三组中α-SMA和FAP的表达水平,以及二者与子宫颈浸润性癌临床病理特征的相关性。结果α-SMA和FAP在正常子宫颈、CIN中不表达,二者在子宫颈浸润性癌中均表达于肿瘤间质成纤维细胞的胞质内,FAP在肿瘤细胞中有少量表达。α-SMA在子宫颈浸润性癌间质中的阳性率为82.7%(81/98),与临床分期、病理分级和淋巴结转移相关(P<0.05);FAP在子宫颈浸润性癌间质中的阳性率为79.6%(78/98),FAP表达与子宫颈浸润性癌的临床分期、病理分级和淋巴结转移相关(P<0.05);α-SMA和FAP均与患者年龄、肿瘤大小无关(P>0.05)。α-SMA和FAP在子宫颈浸润性癌组织标本中表达在区域上有一致性,在表达强度上呈明显正相关。经直线相关分析α-SMA和FAP呈直线相关(r=0.829,P=0.000)。结论α-SMA和FAP在子宫颈浸润性癌生长、浸润和转移方面发挥一定作用,可作为预测子宫颈浸润性癌预后的可靠指标。
Purpose To detect the expression level of a-smooth muscle actin (α-SMA) and fibroblast-activating protein (FAP) in normal cervix, cervical intraepithelial neoplasms (CIN) , and invasive cervical carcinoma, and to analyze the correlation between α- SMA and FAP expressions and clinicopathological parameters, invasion and metastasis and prognosis. Methods Immunohistochemical EliVision two-steps was used to detect the expression level of α-SMA and FAP in 20 cases of normal cervical tissues, 20 cases of ( CIN), 98 cases of invasive cervical cancer. The positive expressions of α-SMA and FAP were compared among the 3 groups of tissues, and the correlation between the positive expression rates of α-SMA and FAP and different clinicopathological characteristics was also analyzed. Results α-SMA and FAP lesions did not express in normal cervix and CIN, however, α-SMA and FAP were expressed in the cytoplasm of tumor interstitial fibroblast of invasive cervical cancer, FAP showed positive expression in a few tumor cells. The positive expression rate of α-SMA in cervical cancer stroma was 82.7% (81/98) , which was related to clinical stage, pathological grade and lymph node metastasis ( P 〈 0. 05 ), the positive expression rate of FAP in cervical cancer stroma was 79. 6% (78/98), which was related to clinical stage, pathological grading, and lymph node metastasis ( P 〈 0. 05 ). α-SMA and FAP expression were not associated with age, tumor size, statistically meaningless ( P 〉 0. 05 ). The expressions of α-SMA and FAP in invasive cervical carcino ma specimens showed consistency on expression area and intensity. The linear correlation analysis showed there was a linear correlation of α-SMA and FAP ( r = 0. 829, P = 0. 000). Conclusion α-SMA and FAP may play a key role in the growth, invasion and metastasis of invasive cervical cancer, which can be used as reliable indicators to predict the malignant biological behavior of cervical carcinoma and prognosis.
