摘要
目的:探讨和研究锁定钢板内固定联合髂骨髓内移植与单纯内固定治疗肱骨近端骨折的疗效。方法:研究对象选取为2013年5月—2014明年9月我院收治的60例肱骨近端骨折患者,随机分为观察组和对照组各30例,两组患者在一般资料对比上显著无统计学差异,对照组采用单纯内固定治疗,观察组则采用锁定钢板内固定联合髂骨髓内移植治疗,对比两组患者的骨折愈合时间、肩臂手功能障碍评分(DASH)、肱骨头高度丢失、Neer评分及并发症情况。结果:观察组患者的骨折愈合时间、DASH评分及肱骨头高度丢失情况均显著优于对照组,差异具有统计学意义(P<0.05);观察组患者的Neer评分优良率为93.3%,显著高于对照组的76.7%,差异具有统计学意义(P<0.05);观察组术后并发症发生率为3.3%,显著低于对照组的36.7%,差异具有统计学意义(P<0.05)。结论:采用锁定钢板内固定联合髂骨髓内移植治疗肱骨近端骨折的疗效明显优于单纯采用锁定钢板内固定治疗,前者能够加速患者的骨折愈合,确保关节功能恢复,减少术后并发症的发生,值得临床推广。
Objective： To study and research the curative effects and contrasts of joint locking plate internal fixation, with bone marrow transplant and simple internal fixation in treatment of proximal humeral fractures. Methods： 60 patients were chosen from our hospital between May, 2013 to Sept. 2014 with proximal humeral fractures and were divided into observation group and control group with 30 cases each, two groups of patients had no significant differences in general data contrast, control group adopts pure internal fixation treatment, observation group adopts bone marrow transplantation with locking plate internal fixation treatment. The two groups patients were after the treatments on the basis of fracture healing time, Jian Bei hand dysfunction score（DASH）, humerus head height loss, Neer scores and complications. Results： observation group of patients had significantly better fracture healing time, DASH score and humerus head height loss situation were than the control group, the difference was statistically significant（P〈0.05）; Neer rating of observation group patients was 93.3%, and was significantly higher than that of control group 76.7%, the difference was statistically significant（P〈0.05）; the incidence of postoperative complications in the observation group was 3.3% which was significantly less than 36.7% of the control group, the difference was statistically significant（P〈0.05）.Conclusion： using joint locking plate internal fixation and bone marrow transplantation in treatment of proximal humeral fractures curative effect is superior to pure with locking plate internal fixation treatment, the former can accelerate fracture healing in patients with, ensure the joint function recovery, reduce the occurrence of postoperative complications, which is worthy of clinical promotion and application.
