摘要
慢性浅表性胃炎以其高发病率位居慢性胃肠病之首,现代医学在诊断、治疗方面均已提出了共识意见,但尚缺乏疗效评价体系;中西医结合及中医药方面制定了相关疗效评价标准,其主要评价指标包括内镜下胃黏膜疗效、胃黏膜组织学疗效、中医症状及证候疗效、生存质量、精神心理评定等,虽具有一定的指导意义,但尚有不足之处,如治疗前后内镜检查与组织学诊断的符合率较低、证候疗效研究缺乏循证医学支持、生存质量量表缺乏中医特色、缺乏安全性及费用-效益比评价、易与功能性消化不良评价指标混淆等。基于此,建议必要时采用放大内镜、共聚焦内镜和定标活组织检查技术使镜下及组织学疗效判定更具针对性,加强证候学的循证研究,创建富有中医特色的生存质量量表并进一步补充完善评价指标,以建立一个更加完善的慢性浅表性胃炎中医药综合疗效评价体系。
Chronic superficial gastritis has the highest incidence in chronic gastrointestinal diseases. Although a consensus has made in the diagnosis and treatment of this disease in western medicine,the evaluation system for therapeutic effects is still unavailable. Relevant efficacy evaluation criteria was established in the field of integrative Chinese and western medicine. The main evaluation indexes include the efficacy of endoscopic gastric mucosa,histological effect of gastric mucosal,traditional Chinese medicine symptoms and syndromes efficacy,quality of life,mental and psychological assessment,etc. Although the evaluation criteria has certain guiding meaning,it still needs to be improved. The problems such as low coincidence rate of endoscopic examination and histological diagnosis before and after treatment,study of syndrome efficacy with few evidence supports,quality of life scale with little of traditional Chinese medicine characteristics,lack of safety and cost benefit ratio evaluation,easily confusion of functional dyspepsia evaluation index,etc. Based on the above,it is suggested that magnifying endoscopy,confocal endoscopy and biopsy techniques are necessary for the purpose of determining the clinical efficacy of endoscopy and histology,evidence based study on syndrome should be strengthened,a quality of life scale with Chinese medicine characteristics should be created,and the evaluation index should be further improved,thus to establish a better evaluation system of TCM comprehensive effect of chronic superficial gastritis.
